Thyroid function in the intensive care unit setting.

Abstract

In sum, there is no convincing evidence yet published supporting the utility of T4 or T3 administration in patients with nonthyroidal illness. The authors recognize that evidence accrued in one disease state may not be applicable to others and that, although these studies are difficult to perform, further large scale prospective studies need to be performed. The issue of T3 treatment will not be resolved satisfactorily until more definitive data are available. Until that time, there may be rare circumstances when a clinician may think it best to treat an individual patient with T4 or T3. For the majority of patients, however, there will be little indication for the administration of thyroid hormones until the potential benefits can be shown to outweigh the risks.

The successful medical management of severe duodenal strictures secondary to eosinophilic gastroenteritis in an infant.

Eosinophilic gastroenteritis is an uncommon condition of unknown etiology that has only been relatively recently reported. Its clinical manifestations range from a mild disease to more severe forms resembling Crohn's disease. Enteric strictures are a rare but recognized complication of this condition. We report a case of eosinophilic gastroenteritis in an infant presenting with severe duodenal strictures that was successfully managed with oral prednisolone. We believe that this is the first reported case of duodenal strictures secondary to eosinophilic gastroenteritis.

[Clinical thinking and decision-making in practice. A new-born with dyspnea].

A 2-day-old girl, born at term after an uneventful pregnancy and delivery, was admitted to the paediatric intensive care unit with dyspnoea and tachypnoea. Misleading interpretations of the radiological investigations suggested a congenital diaphragmatic hernia. The patient underwent laparotomy but a diaphragmatic hernia was not found. Meanwhile the patient developed unexplained pulmonary hypertension and a progressive forward failure of the heart. The differential diagnosis did not take the pulmonary hypertension into account. Finally further investigations led to the diagnosis of a very rare congenital pulmonary vascular anomaly: an absent left pulmonary artery and systemic to pulmonary collateral arteries. The condition was considered inoperable and the patient, whose condition meanwhile had deteriorated markedly, died. This case illustrates (a) that the differential diagnosis must be based on all of the abnormal clinical findings, which should preferably be grouped together under one final diagnosis, and (b) that one must not jump to conclusions--which in this case led to unnecessary diagnostic and therapeutic interventions--, but as long as a diagnosis cannot be established, the diagnostic investigations should not be considered completed.

Diffusion-weighted MR imaging of pleural fluid: differentiation of transudative vs exudative pleural effusions.

The aim of this study was to evaluate the ability of diffusion-weighted MRI in differentiating transudative from exudative pleural effusions. Fifty-seven patients with pleural effusion were studied. Diffusion-weighted imaging (DWI) was performed with an echo-planar imaging (EPI) sequence (b values 0, 1000 s/mm(2)) in 52 patients. The apparent diffusion coefficient (ADC) values were reconstructed from three different regions. Subsequently, thoracentesis was performed and the pleural fluid was analyzed. Laboratory results revealed 20 transudative and 32 exudative effusions. Transudates had a mean ADC value of 3.42+/-0.76 x 10(-3) mm(2)/s. Exudates had a mean ADC value of 3.18+/-1.82 x 10(-3) mm(2)/s. The optimum cutoff point for ADC values was 3.38 x 10(-3) mm(2)/s with a sensitivity of 90.6% and specificity of 85%. A significant negative correlation was seen between ADC values and pleural fluid protein, albumin concentrations and lactate dehydrogenase (LDH) measurements ( r=-0.69, -0.66, and -0.46, respectively; p<0.01). The positive predictive value, negative predictive value, and diagnostic accuracy of ADC values were determined to be 90.6, 85, and 88.5%, respectively. The application of diffusion gradients to analyze pleural fluid may be an alternative to the thoracentesis. Non-invasive characterization of a pleural effusion by means of DWI with single-shot EPI technique may obviate the need for thoracentesis with its associated patient morbidity.
