Abstract

The Multi-Mode Compression (MMC) architecture is a new system for better speech understanding, listening comfort and user acceptance. This study reports on the user tests that have been performed to verify the benefits of the concept and the quality of the fitting process for the individual hearing impaired. The study concentrates on the 4-channel version of a new series of digital instruments with this MMC architecture.

Electrophysiological analysis of cognitive slowing in subjects with mitochondrial encephalomyopathy.

Mitochondrial encephalomyopathies (MEs) are multisystemic inherited disorders affecting tissues with high energy requirement such as the muscle, retina and central nervous system. Progressive external ophthalmoplegia and myopathy are the most common features in adults, and cognitive impairment is rare. In many neurodegenerative disorders, ERPs have been effectively performed to record cognitive slowing on tasks with different amount of information. To analyze the evidence for possible cognitive slowing, a standard auditory oddball paradigm with a button-press response was applied. Participants were 11 non-demented patients affected by mitochondrial encephalomyopathy and 14 age-matched normal controls. This hypothesis was tested using two tasks of different difficulty (pure tone vs. phonetic stimuli). Reaction time (RT), performance (P) and event-related potentials (ERPs) were measured. RT and P were not significantly different between the groups. Patients showed significantly increased N2 latency and reduced P3 amplitude on both tasks. No difference was found in pure tone and phonetic task conditions. Results were interpreted as electrophysiological signs of cognitive slowing--particularly in relation to stimulus evaluation--irrespective of sensory problems, response selection and cognitive load. These findings suggest that in ME patients, there may be a possible dysfunction of neural mechanisms underlying cognitive events and ERP generation.

Deficits in auditory brainstem pathway encoding of speech sounds in children with learning problems.

Auditory brainstem responses were recorded in normal children (NL) and children clinically diagnosed with a learning problem (LP). These responses were recorded to both a click stimulus and the formant transition portion of a speech syllable /da/. While no latency differences between the NL and LP populations were seen in responses to the click stimuli, the syllable /da/ did elicit latency differences between these two groups. Deficits in cortical processing of signals in noise were seen for those LP subjects with delayed brainstem responses to the /da/, but not for LPs with normal brainstem measures. Preliminary findings indicate that training may be beneficial to LP subjects with brainstem processing delays.

Alterations in the auditory startle response in Fmr1 targeted mutant mouse models of fragile X syndrome.

Fragile X syndrome results from inadequate production of the fragile X mental retardation protein (FMRP). Mice with a mutation targeted to the Fmr1 gene lack FMRP and thus are a valuable animal model for studying the behavioral and neural phenotype of this human disorder. Mice of two genetic backgrounds containing the Fmr1 mutation, C57BL/6J (C57-KO) and an F1 hybrid (C57BL/6J mutant x FVB/NJ; F1-KO) did not differ from control mice in behavior in the elevated plus maze or the open field. Both the C57-KO and F1-KO mice exhibited greater startle responses than normal mice to low intensity (80 dB) white noise bursts and decreased responses to high intensity (120 dB) white noise bursts. These behavioral alterations appear to be specific to the Fmr1 mutation since they are present on both genetic backgrounds. Furthermore, the mice lacking FMRP resemble individuals with fragile X syndrome in their increased sensitivity to low intensity auditory stimuli. These findings should prove useful in determining how the absence of FMRP alters the brain and behavior, and in testing potential treatments for fragile X syndrome.
