Small nasal defects.

Abstract

The nose occupies an important feature of a person's central face. The size, shape, and proportions of the various characteristics of the face provide a visual basis suggesting the character of the person. Scars, asymmetries, and any abnormalities of the central face are easily recognizable and noted by even a casual observer. Over the centuries, therefore, much attention has been paid to reconstruction of facial and nasal abnormalities of both acquired and congenital etiologies. The purpose of this article is to address the reconstruction of smaller defects of the nose, encompassing surface areas of 1.5 to 2 centimeters.

Laryngotracheal reconstruction using prefabricated and preconditioned composite radial forearm free flaps. A report of two cases.

OBJECTIVE: Total or subtotal laryngectomy performed as surgical treatment of locally advanced laryngeal tumors requires a permanent laryngostomy or tracheostomy, and are both associated with severe and lifelong disability. The hitherto published reconstructive methods for the rehabilitation of patients after subtotal anterior laryngotracheal resection of extended laryngotracheal malignancies have not been thoroughly convincing. METHODS: We present a successful method of extended laryngotracheal reconstruction that combines modern surgical and biomedical techniques. RESULTS: A stable three-dimensional anterior neolarynx was prefabricated, including the use of vacuum-assisted prelamination and preconditioning of a composite radial forearm free flap, which was then transferred to the neck and vascularized by microvascular anastomosis to the cervical vessels. This reconstructive procedure permitted the restoration of a stable laryngotracheal conduit in two cases, with the subsequent successful closure of the tracheo-laryngostomy. CONCLUSION: In the reported cases the pre- or post-operative irradiation did not compromise the success of reconstruction. Not only did our technique facilitate the rehabilitation of patients suffering from drawbacks of the laryngo-tracheofissure, but it also assisted in avoiding total laryngectomy in cases of advanced anterior laryngeal cancer by extending the indications for subtotal anterior laryngeal resections followed by reconstructive surgery.

Development of a spray congealing process for the preparation of insulin-loaded lipid microparticles and characterization thereof.

A spray congealing process for the preparation of protein-loaded microparticles was developed. The influence of the process parameters atomization pressure and spraying temperature on particle size and process yield was investigated by experimental design. The employed spray congealing technique enabled the production of microparticles with yields ranging from 79% to 95% and median particle sizes (d(0.5)) from 182.2 to 315 microm. Insulin lipid microparticles could be prepared without any loss of insulin during the preparation process and the protein stability was not affected by the spray congealing process as investigated by HPLC-MS analysis. The stability of insulin encapsulated in lipid microparticles under release conditions over 28 days was assessed by investigating the residual insulin content. Starting after 3 days of release, a continuous increase of desamidoinsulin in the remaining particles of up to 7.5% after 28 days was observed. An additional degradation product was detected by HPLC and HPLC-MS analysis and identified as a covalent insulin dimer by MALDI-ToF. The microparticles did not show a burst release and testing the insulin lipid microparticles in a fibrin gel chondrocyte culture revealed that the released insulin was bioactive and had a significant effect on chondrocyte extracellular matrix production.

Morphologic, experimental-comparative investigation as an identification of the injuring instrument method.

Aiming to identify the injuring tool characteristics and the tool itself morphologic, experimental-comparative investigations of the skin wound, rib and cartilage injuries taken during the autopsy are performed. During 1995-2004, 489 investigations were performed for this purpose. In 418 cases, knives were submitted for identification of the specific injuring tool (in total-835 knives). In 205 cases the investigation included not only skin wounds, but also the injured rib cartilages. Identification investigations were performed by investigating both the skin wounds morphologic characteristics and dynamic traces-trails in the rib cartilage tissue left by the micro relief of the knife blade edge. In the case of the investigated and experimental skin wounds characteristics coincidence the experimental and comparative dynamic traces investigation was performed when the traces were suitable for the tool identification purposes. In the case of the investigated and experimental skin wounds, dynamic traces coincidence, the totality of the coincided characteristics was considered individual. In those cases, the conclusion included the fact that the injury had been made by a particular knife. According to our data during 1995-2004 in 23 cases-15.9% (5.5% out of the total investigated cases), the knife identification was based on the skin wound characteristics and dynamic traces in the rib cartilage tissue. In our opinion, the dynamic traces in the rib cartilage tissue investigations supplement the identification field and are valuable in the tool identification. In 11 (2.6% out of the total investigated cases) cases knives were identified only by the skin wounds morphologic characteristics, the ribs being not injured or dynamic traces being not suitable for the tool identification.
