[Weak opioids].

Abstract

Weak opioids have been used as analgesics in cancer patients with moderate to severe chronic pain. Codeine is one of the weak opioids which is assigned as a representative analgesic of the 2nd ladder-drugs for the treatment of cancer pain by WHO cancer pain relief programme. Analgesic effect of codeine is recognized when it is administered with 20 mg p.o. or more. Clinical ceiling effect of codeine is seemed to be 200-300 mg/day, although it is described as 600 mg/day in some textbooks. Side effects of codeine are same as those of morphine, therefore, drugs for the side effects should be given to the patients simultaneously when codeine is administered.

Caroverine inhibits the conditioned place aversion induced by naloxone-precipitated morphine withdrawal in rats.

Comparable to the anti-craving compound acamprosate, caroverine reduces alcohol withdrawal symptoms by an antagonism at ionotropic glutamate receptors and a blockade of calcium channels. The present study examines the effect of caroverine in a craving model, in which acamprosate has proved effective before. A place aversion was induced by a conditioned morphine withdrawal by administration of the opioid antagonist naloxone (0.1 mg/kg, s.c.) 5-6 days after the subcutaneous implantation of a morphine pellet in rats. Testing in the drug-free state, the acquisition of a conditioned aversion against the naloxone-associated cues was inhibited by pretreatment with caroverine (5 mg/kg, i.p.). This result corroborates the involvement of excitatory glutamate and calcium in the development of conditioned withdrawal and craving. It offers further evidence for the hypothesis that these neuronal systems are altered during withdrawal in a similar way by ethanol and opiates.

Repeated exposures to heroin and/or cadmium alter the rate of formation of morphine glucuronides in the rat.

After absorption, heroin is transformed into mono-acetyl-morphine and then into morphine. Morphine, in turn, is metabolized to morphine-3-glucuronide (M3G), an inactive compound, and morphine-6-glucuronide (M6G), a potent opioid agonist. Thus, changes in the rate of formation of M6G may alter the pharmacological consequences of a treatment with heroin or morphine. In this study, we investigate the effect of repeated exposures (10 daily i.p. injections) to heroin, morphine, cadmium (which has been previously shown to inhibit M3G formation in vitro), or heroin + cadmium on morphine glucuronidation both in vivo and ex vivo (i.e., microsomal preparation obtained from rats treated in vivo). Repeated heroin (2.5, 5.0, and 10 mg/kg) increased plasma levels of M6G (which was undetectable in all other groups) and reduced those of M3G. Also, the microsomal preparations obtained from the liver of repeated heroin rats, when incubated with morphine, yielded significant amounts of M6G (which was undetectable in all other groups) and decreased levels of M3G relative to the control groups. These effects were reversible upon discontinuation of heroin administration. In contrast, repeated morphine (10, 20, and 40 mg/kg) only slightly reduced M3G formation at the dose of 40 mg/kg. Repeated cadmium (5, 15, and 45 microg/kg) reduced the rate of M3G formation without inducing M6G synthesis. The effects of the repeated coadministration of heroin (10 mg/kg) and cadmium (15 microg/kg) were virtually identical to those of repeated heroin alone. In summary, repeated exposure of rats to heroin can shift morphine glucuronidation toward the formation of the active metabolite M6G.

Advances in palliative care relevant to the wider delivery of healthcare.

The availability of a variety of opioids, together with the discovery of new uses for old drugs (such as ketamine), assists individualised pain management in palliative care. Experience in palliative care provides reassurance that the effective use of opioids and sedatives does not accelerate the approach of death. In taking patient histories, recognising the spiritual component of life experience enlarges the focus of care. Interdisciplinary care brings many different insights to care situations in a prospective and cooperative way. Models of bereavement care established in palliative care units deserve wider implementation in medicine. An "experiential" model of medical student education encourages a focus on the whole experience of patients and their journey with their carers.
