Abstract

The effective reduction of medical errors depends on an environment of safety for patients in both clinically-based and systems-oriented arenas. Formal teamwork training is proposed as a systems approach that will achieve these ends. In a study conducted by Dynamics Research Corporation, weaknesses and error patterns in Emergency Department teamwork were assessed, and a prospective evaluation of a formal teamwork training intervention was conducted. Improvements were obtained in five key teamwork measures, and most importantly, clinical errors were significantly reduced.

The use of classroom training and simulation in the training of medical responders for airport disaster.

There is a dire need to have complementary form of disaster training which is cost effective, relatively easy to conduct, comprehensive, effective and acceptable. This will complement field drills training. A classroom-based training and simulation module was built by combining multiple tools: Powerpoint lectures, simulations utilising the Kuala Lumpur International Airport (KLIA) schematic module into 'floortop' model and video show of previous disaster drill. 76 participants made up of medical responders, categorised as Level 1 (specialists and doctors), Level 2 (paramedics), Level 3 (assistant paramedics) and Level 4 (health attendants and drivers) were trained using this module. A pre-test with validated questions on current airport disaster plans was carried out before the training. At the end of training, participants answered similar questions as post-test. Participants also answered questionnaire for assessment of training's acceptance. There was a mean rise from 47.3 (18.8%) to 84.0 (18.7%) in post-test (p<0.05). For Levels 1, 2, 3 and 4 the scores were 94.8 (6.3)%, 90.1 (11)%, 80.3 (20.1)% and 65 (23.4)% respectively. Nevertheless Level 4 group gained most increase in knowledge rise from baseline pre-test score (51.4%). Feedback from the questionnaire showed that the training module was highly acceptable. A classroom-based training can be enhanced with favourable results. The use of classroom training and simulation effectively improves the knowledge of disaster plan significantly on the back of its low cost, relatively-easy to conduct, fun and holistic nature. All Levels of participants (from specialists to drivers) can be grouped together for training. Classroom training and simulation can overcome the problem of "dead-document" phenomenon or "paper-plan syndrome".

The results of cosmic radiation in-flight TEPC measurements during the CAATER flight campaign and comparison with simulation.

The European-Commission-supported project DOSMAX (Dosimetry of Aircrew Exposure to Radiation During Solar Maximum) was aimed at measuring aircrew exposure to cosmic radiation on-board the aircraft during solar maximum. During a dedicated international comparison mission (Co-ordinated Access to Aircraft for Transnational Environmental Research; CAATER) different measurement techniques have been compared by six European institutes (Results of the CAATER Mission, DOSMAX Meeting, Dublin, June 2004). In this paper, we present the tissue-equivalent proportional counter (TEPC) measurements carried out by ARC Seibersdorf research (ARCS), Austria, and Institut de Radioprotection et de Sûreté Nucléaire (IRSN), France, together with a comparison with simulation results under the same conditions. The whole flight campaign consists of four different in-flight investigations performed at two different geographical positions at 12.2 km (FL 400) and 9.8 km (FL 320). One location was chosen above Rome (42 degrees North, 12 degrees East), Italy, for high cut-off rigidity (6.4 GV) and the second above Aalborg (57 degrees North, 10 degrees East), Denmark, for low cut-off rigidity (1.8 GV). The TEPC measurements are presented in terms of absorbed dose and ambient dose equivalent as well as microdosimetric spectra as a function of lineal energy. For the same conditions of the CAATER flights the response of the TEPC has also been simulated by using the Monte Carlo Transport Code FLUKA (version 2003). The results from simulations are compared with measurements and they show a reasonable agreement.

Uptake of HNO3 on hexane and aviation kerosene soots.

The uptake of HNO(3) on aviation kerosene (TC-1) soot was measured as a function of temperature (253-295 K) and the partial pressure of HNO(3), and the uptake of HNO(3) on hexane soot was studied at 295 K and over a limited partial pressure of HNO(3). The HNO(3) uptake was mostly reversible and did not release measurable amounts of gas-phase products such as HONO, NO(3), NO(2) or N(2)O(5). The heat of adsorption of HNO(3) on soot was dependent on the surface coverage. The isosteric heats of adsorption, Delta(0)H(isosteric), were determined as a function of coverage. Delta(0)H(isosteric) values were in the range -16 to -13 kcal mol(-1). The heats of adsorption decrease with increasing coverage. The adsorption data were fit to Freundlich and to Langmuir-Freundlich isotherms. The heterogeneity parameter values were close to 0.5, which suggested that a HNO(3) molecule can occupy two sites on the surface with or without being dissociated and that the soot surface could be nonuniform. Surface FTIR studies on the interaction of soot with HNO(3) did not reveal formation of any minor product such as organic nitrate or nitro compound on the soot surface. Using our measured coverage, we calculate that the partitioning of gas-phase nitric acid to black carbon aerosol is not a significant loss process of HNO(3) in the atmosphere.
