Efficient diagnostic test sequence: applications of the probability-modifying plot.

Abstract

Excessive diagnostic testing rarely leads to complete certainty of the disease status of a patient. Because of constraints of health care budgets, physicians should be aware of the diminishing marginal gain of each subsequent test in a testing strategy related to costs. We developed the probability-modifying plot to determine economically efficient sequence of testing without losing clinical effectiveness. Two applications of the model are presented: diagnosis of Helicobacter pylori and diagnosis of benign prostatic hyperplasia. The probability-modifying plots are drawn to visualize and calculate the economic efficiency of all possible test sequences, meaning that although the different test sequences are equally accurate, some have even lower costs than others.

Evidence-based decision making in action: Part 2--evaluating and applying the clinical evidence.

This is the second of a two-part series addressing the use of evidence-based decision making (EBDM) in the use of home bleaching. In Part 1, a case scenario demonstrated the skills involved in (1) structuring a clinical question and (2) conducting an online search using PubMed.1 Part 2 demonstrates the third and fourth steps in the EBDM process, i.e., (3) critical appraisal to assess the validity of a study and (4) applying that information to clinical decision making. This 4-step approach to EBDM recognizes that clinicians can never be completely current with all conditions, medications, materials, and products. Thus, EBDM provides a mechanism for addressing these gaps in knowledge in order to provide the best care possible.

Decision support for the selection of an appropriate in-lake measure to influence the phosphorus retention in sediments.

Many in-lake measures which aimed to influence the phosphorus retention in lake sediments have failed to improve the trophic state of the lakes. The present paper introduces a systematic approach to select an appropriate in-lake measure. Before selecting an in-lake measure the goal of the measure should be defined, the problems of the lake must be identified, and the probability of success must be estimated. The proposed decision support consists of two parts. Part A, pre-selection, excludes inappropriate measures. It uses six criteria, which are mainly based on a simple mass-balance model, and the targets of restoration. The criteria describe the magnitude of the external versus the internal phosphorus loading, the dynamics of the internal load, and the lake morphometry. Each measure is weighted differently with respect to importance and suitability by specific quantified limits. Part B, selection, uses qualitative criteria, which are specific to the measure in question. Checking these criteria will help to select a measure with a low risk of failure. The suggested decision support is illustrated in flow charts and exemplified by Lake Arendsee in Germany.
