Managing practice statistics for practice success.

Abstract

The incorporation of computers into the dental practice has enabled the dentist and staff to quickly access information vital to monitoring practice growth and vitality. Despite the fact this information is available at the click of a mouse, the information available to the dentist and staff is often ignored. Even if an effort is made to assess the data, the dental team is clueless on how to use it to control changing economics in the practice. This is accomplished by charting statistical data collected by the dentist and staff. Compilation of this data enables the dentist and staff to recognize trends that will affect the growth and profitability of the practice and develop strategies that reinforce positive trends and reverse negative trends. This paper discusses the value of using data as a tool for practice management.

Adjusting for partially missing baseline measurements in randomized trials.

Adjustment for baseline variables in a randomized trial can increase power to detect a treatment effect. However, when baseline data are partly missing, analysis of complete cases is inefficient. We consider various possible improvements in the case of normally distributed baseline and outcome variables. Joint modelling of baseline and outcome is the most efficient method. Mean imputation is an excellent alternative, subject to three conditions. Firstly, if baseline and outcome are correlated more than about 0.6 then weighting should be used to allow for the greater information from complete cases. Secondly, imputation should be carried out in a deterministic way, using other baseline variables if possible, but not using randomized arm or outcome. Thirdly, if baselines are not missing completely at random, then a dummy variable for missingness should be included as a covariate (the missing indicator method). The methods are illustrated in a randomized trial in community psychiatry.

A space-time permutation scan statistic for disease outbreak detection.

BACKGROUND: The ability to detect disease outbreaks early is important in order to minimize morbidity and mortality through timely implementation of disease prevention and control measures. Many national, state, and local health departments are launching disease surveillance systems with daily analyses of hospital emergency department visits, ambulance dispatch calls, or pharmacy sales for which population-at-risk information is unavailable or irrelevant. METHODS AND FINDINGS: We propose a prospective space-time permutation scan statistic for the early detection of disease outbreaks that uses only case numbers, with no need for population-at-risk data. It makes minimal assumptions about the time, geographical location, or size of the outbreak, and it adjusts for natural purely spatial and purely temporal variation. The new method was evaluated using daily analyses of hospital emergency department visits in New York City. Four of the five strongest signals were likely local precursors to citywide outbreaks due to rotavirus, norovirus, and influenza. The number of false signals was at most modest. CONCLUSION: If such results hold up over longer study times and in other locations, the space-time permutation scan statistic will be an important tool for local and national health departments that are setting up early disease detection surveillance systems.
