Specialist nurse network improves MS practice.

Abstract

This article looks at the development of a group dedicated to meeting the clinical supervision and networking needs of multiple sclerosis nurse specialists. It describes the evolution of the group, the structure and content of group meetings and how they have both improved practice and reduced the isolation of MS specialists who work independently. The nurses involved have found that working collaboratively and developing an interactive network has not only significantly affected their clinical practice but also helped to address work-related stress.

An asset-based community initiative to reduce television viewing in New York state.

BACKGROUND: Childhood obesity is an epidemic. Addressing this problem will require the input of many sectors and change in many behaviors. The "community" must be part of the solution, and the solution must be constructed on existing assets that lend strength to positive environmental change. OBJECTIVE: To catalyze an established asset-based community partnership to support efforts to reduce television viewing time by developing and providing alternative activities as part of a broader, 3-year study to reduce childhood obesity among preschool-aged children in rural, upstate New York. METHOD: Asset mapping was utilized to compile an inventory of individual and community strengths upon which a partnership could be established. Facilitated focus group sessions were conducted to better understand childcare environmental policies and practices, and to guide changes conducive to health and fitness. Planning meetings and targeted outreach brought key stakeholders together for a community-participatory initiative to support positive environmental change. RESULTS.: The partnership planned and initiated an array of after-school and weekend community activities for preschool-aged children and their families in the weeks preceding, during, and following a designated 'TV Turn-off' week in April, 2004 and March, 2005. CONCLUSION: Methods of asset-based community development are an effective way to engage community participation in public health initiatives.

End-stage renal disease use in hurricane-prone areas: should nephrologists increase the utilization of peritoneal dialysis?

Hurricane Katrina resulted in massive destruction of the gulf coast of the United States in 2005. In the immediate aftermath, displaced dialysis patients required urgent hemodialysis or additional peritoneal dialysis (PD) supplies. Massive damage to the health care infrastructure in these communities disrupted dialysis services for several months. As a result of this event and subsequent storms during the 2005 Atlantic hurricane season, many decisions regarding future services to dialysis patients in hurricane prone communities (HPCs) need to occur. Nephrologists, dialysis nurses, dialysis providers, and patients need to discuss the ramifications of and types of continued dialysis services in HPC. Nephrologists should encourage PD, and efforts to educate on other renal replacement therapies including PD and transplant should occur. With the potential for interruption of electrical, sewerage, and water services, more patients should consider PD. Recovery from future events begins with appropriate disaster planning. Many questions are considered and need answering in planning for dialysis services in HPC and other communities subject to natural disasters. This summary provides the basis to begin discussions when planning for dialysis services in communities prone to natural disasters.

An interagency network perspective on HIV prevention.

OBJECTIVE: The objective of this study was to describe the network structures of agencies and individuals engaged in HIV prevention in a North Carolina county. STUDY DESIGN: The authors conducted a cross-sectional study of interconnected agencies and individuals identified through snowball sampling. Participants were interviewed face to face with a structured questionnaire about interactions with others in the network. Network characteristics such as density and centralization were computed with UCINET software. RESULTS: The network of 11 agencies had an information exchange density of 14%. Exchanges of clients and funds were even more rare. The network of 17 individuals influential in HIV prevention was better connected with a density of 42%. CONCLUSIONS: The HIV prevention agencies in the county were not functioning as a network and thus were not benefiting the community to their full potential. Any connectedness between the agencies may be attributable to the better connections between influential individuals, several of whom worked for the agencies in the agency network.
