The Charcot library.

Abstract

The Charcot library was officially created at the Salpêtrière Hospital in 1907 after the donation of the private library of Jean-Martin Charcot, which Dr. Charcot had accumulated progressively throughout his professional career. Increased by several other endowments (the most important being the collection of the Resident's Library, begun in 1886) and other private donations, the library became officially affiliated with the Paris VI University in 1985. It now functions as an entity oriented toward the neurosciences and offers a large number of 19th-century books (including all of Charcot's manuscripts) and more recent documents. Digitization of the most classic holdings, including atlases, will soon make them accessible through the library's web site.

Novel insights into tomato leaf curl New Delhi virus introduction and evolution in Southeastern France using an advanced long-read sequencing workflow.

The Mediterranean population of tomato leaf curl New Delhi virus (ToLCNDV-ES) is characterized by a high genetic uniformity, distinguishing it from its Asian counterparts. ToLCNDV-ES is thought to have a monophyletic origin, likely resulting from a single recombination event, prior to its spread throughout the Mediterranean region. Following its first detection in southeastern France in 2020, ToLCNDV-ES re-emerged in France in 2022. Our analysis based on advanced long-read sequencing, circular DNA profiling, and phylogeny indicates both local persistence of French ToLCNDV-ES and multiple independent introduction events. Signatures of positive selection were identified in French ToLCNDV-ES populations, whereas no clear evidence of recombination was found. Bayesian time-structured phylogenetic analyses suggest that introductions in France occurred between 2018 and 2021 from the major ToLCNDV-ES clade, while several Italian ToLCNDV-ES isolates diverged prior to the virus introduction in the Mediterranean basin. Overall, this study demonstrates the value of an optimized long-read sequencing approach for resolving circular DNA virus diversity, and sheds light on the complex evolutionary history of ToLCNDV-ES in the Mediterranean Basin, particularly in southeastern France.

Marcel Proust's lifelong tour of the Parisian Neurological Intelligentsia: from Brissaud and Dejerine to Sollier and Babinski.

In Search of Lost Time, the main novel of Marcel Proust (1871-1922) gives prominence to medicine, especially to neurology. Proust possessed excellent medical knowledge and maintained lifelong contact with neurologists. From 1881 onward, he experienced recurrent attacks of asthma, a condition which, at the time, was considered belonging to 'neurasthenia'. Marcel's father, Adrien Proust, was a famous physician who had written papers on stroke, aphasia, hysteria and neurasthenia, and who introduced his son to Charcot's pupil, Edouard Brissaud, the founder of the Revue Neurologique. Three years later, Brissaud published a landmark book on asthma with a preface by Adrien Proust. In 1905, when Proust intended to undergo a 'cure' in order to improve his asthma and other symptoms, he first considered treatment by Jules Dejerine, who was to become Charcot's second successor. He also considered two Swiss physicians who had studied with Charcot and Vulpian: Henry Auguste Widmer, founder of the Clinique Valmont above Montreux, and Paul Dubois, a schoolmate of Dejerine, who practiced in Berne. Brissaud recommended Paul Sollier, under whose care Proust followed a 6-week 'isolation cure'; Sollier, along with Babinski, was considered the cleverest of Charcot's followers. He had studied memory extensively, in particular affective memory, which caused him to reject Bergson's theories and now makes his work a major precursor. Sollier attempted to trigger 'emotional revivals' (reviviscences), 'reproducing the entire state of the personality of the subject at the time of the initial experience'. This concept was integrated by Proust into his novel, with emphasis on 'involuntary memory'. Proust's last neurologist was Joseph Babinski, whom he consulted repeatedly because he feared becoming aphasic, like his mother. Proust's unusual life journey with the most celebrated neurologists of his time highlights aspects of his literary work and also provides a unique perspective on the neurological intelligentsia at the turn of the 19th century.

The current state of adult mental health care in France.

Since the 1960s, French public mental health services are organised in "sectors", each sector catering for a mean population of 54,000 inhabitants. Although this organisation was aimed at insuring equal access to care whatever the place of residence, there are still huge disparities in number of staff and bed resources from one sector to another. The reduction in the number of hospital beds started later in France than in most other European countries, and was really effective in the 1990s. In 2000, there were 9.4 beds for 10,000 inhabitants aged over 20 years. Hospital-based care has still an overwhelming importance, and is associated with a marked under-development of community services and lack of sheltered housing for the most disabled patients. With more than 13,000 registered psychiatrists in France, the density of psychiatrists is one of the highest in the world. However, French psychiatry has currently to face a structural crisis due to the reduction in public health budgets, as well as to the reduction of 30% in the number of French psychiatrists over the next two decades. The numerous national programmes aimed at renovating French mental health services, published over the last decade, have not yet kept their promises.
