Linear abdominal trauma.

Abstract

Three cases of blunt abdominal trauma are presented to exemplify the mechanism of trauma and the problems of diagnosis associated with any linear blow to the abdomen. The mechanisms of visceral injury are reviewed, and special attention is directed to the abdominal wall injury that can be present in these patients. This injury has special implications in directing the operative approach and repair. An unusual aortic occlusion is described which is peculiar to this type of injury.

[CT of intestinal injuries following blunt trauma].

We retrospectively reviewed 53 CT scans in 42 patients with surgically proven intestinal injury following blunt abdominal trauma. Free air and localized low density fluid were specific signs of intestinal injury. Free peritoneal fluid without a known source, thickened bowel wall, and thickened mesentery were non-specific ones. Specific and non-specific findings were demonstrated in six (22%) and 23 (85%) of 27 CT scans performed within four hours following trauma, and 19 (73%) and 26 (100%) of 26 CT scans performed after four hours, respectively. CT is useful for the diagnosis of blunt intestinal injuries, but early diagnosis is difficult because of the lack of specific signs.

[Complicated aneurysm of the splenic artery].

For 2 1/2 years after sustaining blunt trauma to the abdomen a 51-year-old woman had experienced episodes of pain, recently with throbbing, in the left upper quadrant. A smooth, circumscribed, painless tumour filling the left upper and mid-abdomen was palpable under the left costal margin. Ultrasound revealed a 12 x 11 cm space-occupying mass with cystic-fluid components, at first interpreted as a blood-containing pseudocyst. Computed tomography demonstrated a connection with the splenic artery. Digital subtraction angiography established the diagnosis of an aneurysm of the splenic artery arising immediately beyond its origin from the coeliac trunk. Splenectomy and resection of the aneurysm were performed. Total removal of the aneurysm was not undertaken because the dorsal aspect of the aneurysm wall was very thick and fused with the retroperitoneum. The postoperative course was unremarkable. The genesis of splenic aneurysm often remains unexplained. Its occurrence after blunt abdominal trauma is exceedingly rare.
