Sensomotoric diabetic polyneuropathy is often associated with damage to autonomic nerve fibers. In the case of the lower extremity, this manifests in the form of a sudomotoric and/or vasomotoric disorder. This in turn is believed to be closely related to the development of neurotrophic ulcerations of the foot. The sudomotoric disorder leads to anhidrosis of the skin of the foot, leading to the formation of rhagades and fissures. Neurovascular problems with disordering of microvascular perfusion and trophic disturbances then become risk factors for foot ulcers, in particular when skin traumata occur in addition. Various methods for identifying an autonomic function disturbance in the lower limb are based either on the measurement of sweat secretion or on the investigation of peripheral microvascular function. A purposeful assessment of risk and prevention of neurotrophic damage in the lower limb is possible only on the basis of a rigorous differential diagnostic work-up of the diabetic polyneuropathy.
Headache syndromes can be associated with focal neurological symptoms or signs. Good knowledge of primary headaches, a detailed history and a thorough clinical examination are prerequisites for their differential diagnosis. The neurological symptoms produced by the migraine aura are the most characteristic and recognisable. However, structural lesions, such as vascular malformations, can produce similar symptoms to migraine with aura, which highlights that paraclinical investigations are necessary in most patients with headache and focal neurological symptoms. In this review, we provide an overview of the differential diagnosis of the most common headache disorders with focal neurological symptoms or signs to refresh the practising neurologist's differential diagnostic knowledge for the clinical situation and to aid the teaching of neurology residents.
We examined the possible role of autonomic activity in Huntington's disease (HD) during a risky decision making task. Skin conductance responses (SCRs) of 15 HD participants and 16 healthy controls were measured while they performed a computerized version of the Simulated Gambling Task (SGT). The results replicated our previous finding of a performance decrement in HD, and showed that HD was associated with an altered pattern of SCRs during the risky decision task. Specifically, the healthy controls produced increased SCRs following selections from the disadvantageous decks and following losing selections. In contrast, the SCRs of the HD group did not differentiate between wins and losses. These findings indicate a reduced impact of loss on decision-making processes under risky conditions in HD.
