Comparison of predictive values of two diagnostic tests from the same sample of subjects using weighted least squares.

Screening and diagnostic tests are important in disease prevention or control. The predictive values of positive and negative (PPV and NPV) test results are two of four operational characteristics of a screening test. We review an existing method based on the generalized estimating equation (GEE) methodology for comparing predictive values from the same sample of subjects and propose two Wald test statistics derived from the weighted least squares (WLS) method for the analysis of categorical data. Using these results, we propose sample size calculation formulae for this problem. Simulation studies are conducted to compare the performances of the two Wald test statistics (one based on the difference of two PPVs or NPVs, another based on the logarithm of the ratio of two PPVs or NPVs) and the score/Wald test statistic derived from GEE. We recommend using the difference-based WLS approach.

Effects of iodinated contrast media on blood and endothelium.

The aim of the study was to assess the effects of iodinated contrast media on blood components and endothelium based on experimental and clinical studies and to produce clinically relevant guidelines for reducing thrombotic and hematologic complications following the intravascular use of contrast media. A report was drafted after review of the literature and discussions among the members of the Contrast Media Safety Committee of the European Society of Urogenital Radiology. The final report was produced following discussion at the 12th European Symposium on Urogenital Radiology in Ljubljana, Slovenia (2005). Experimental data indicate that all iodinated contrast media produce an anticoagulant effect and that this effect is greater with ionic contrast media. Several of the in vitro and experimental in vivo studies on haematological effects of contrast media have not been confirmed by clinical studies. Low- or iso-osmolar contrast media should be used for diagnostic and interventional angiographic procedures, including phlebography. Meticulous angiographic technique is the most important factor for reducing the thrombotic complications associated with angiographic procedures. Drugs and interventional devices that decrease the risk of thromboembolic complications during interventional procedures minimize the importance of the effects of contrast media.

Renovascular disease imaging: today more than ever the invasive cardiologist may make the difference.

Doppler ultrasound (US), computed tomography (CT) and magnetic resonance imaging (MRI) are gaining wide acceptance as gold standard for diagnosing peripheral artery disease at thoracic and abdominal aorta level and infrainguinal arteries. A different case is that of renal artery stenosis (RAS): noninvasive techniques, CT and MRI in particular, failed to become the new gold standard. Image interpretation and artifacts seems to invalidate MRI and CT sensitivity and specificity that remain quite low. The debacle of noninvasive imaging of renal arteries may have important reflexes on the invasive cardiology practice. In the light of the results of noninvasive tests for renal artery stenosis, the angiography of renal vessels performed at the time of coronary artery angiography may became a new tool for invasive professional to optimize the diagnosis of renal artery stenosis at least in patients scheduled for coronary angiography. The invasive cardiologist, today more than ever, has the chance to really impact the diagnosis and therapy of patients with renovascular disease and the prognosis of patients with both coronary artery and renal artery disease.
