A practical guide to anaphylaxis.

Abstract

Anaphylaxis is a life-threatening reaction with respiratory, cardiovascular, cutaneous, or gastrointestinal manifestations resulting from exposure to an offending agent, usually a food, insect sting, medication, or physical factor. It causes approximately 1,500 deaths in the United States annually. Occasionally, anaphylaxis can be confused with septic or other forms of shock, asthma, airway foreign body, panic attack, or other entities. Urinary and serum histamine levels and plasma tryptase levels drawn after onset of symptoms may assist in diagnosis. Prompt treatment of anaphylaxis is critical, with subcutaneous or intramuscular epinephrine and intravenous fluids remaining the mainstay of management. Adjunctive measures include airway protection, antihistamines, steroids, and beta agonists. Patients taking beta blockers may require additional measures. Patients should be observed for delayed or protracted anaphylaxis and instructed on how to initiate urgent treatment for future episodes.

Allergenicity of airborne cyanobacteria Phormidium fragile and Nostoc muscorum.

In recent times, airborne microorganisms and their constituents have become prominent safety and health concern. Ongoing climatic changes coupled with unwarranted human activities have significantly deteriorated the ambient air quality. In certain environments, airborne algae contribute significantly to the total biological load of the atmosphere, hitherto dominated by bacteria and fungi. Present study was aimed to investigate the allergenic potency of two frequent viable algal forms i.e., Phormidium fragile and Nostoc muscorum found in the atmosphere of Varanasi City, India. To test the allergenic potency, crude extracts of these strains were subjected to intra-dermal allergy test and subsequent leukocyte counts, which revealed their allergenic nature. Both the species varied in their allergenic potency. N. muscorum appeared to be more allergenic than P. fragile. However, when the allergens were mixed in equal amounts, the severity of allergenicity increased significantly. A limited pattern of cross-reactivity between the species was also evident.

Determination of retention indices in constant inlet pressure mode and conversion among different column temperature conditions in comprehensive two-dimensional gas chromatography.

A method to determine the second dimensional real retention time, dead times on both dimensions and retention indices in constant inlet pressure mode was developed in comprehensive two-dimensional gas chromatography. At the same time, the conversion of GC x GC retention indices among different column temperature conditions were also conducted based on some thermodynamics parameters. The calculation accuracies are better than 1.0 retention index unit. Furthermore, a retention index database was developed and used to identify the compounds in a cigarette essential oil sample. It showed that identification by the database was of close agreement with by time-of-flight mass spectrometry, and some isomers could also be distinguished based on the retention index database.

Immunoregulation by targeting T cells in the treatment of allergy and asthma.

T-cell responses to allergens are crucial in determining the choice between health and disease. Th2 responses drive synthesis of IgE and the recruitment, maturation, survival and effector function of accessory cells such as eosinophils, basophils and mast cells. Allergen-specific strategies for targeting T-cell responses in established allergic diseases have been employed with success for almost a century. Recently, new insight into the mechanism of action of such approaches has revealed modulation of the delicate and complex regulatory mechanisms including suppression of Th2 responses through antagonistic Th1 responses and regulation through IL-10 and TGFbeta production.
