[Resource distribution for the Basic Care Plan with equity approach, Bogotá 2002].

Abstract

OBJECTIVE: To apply a technique that will guide financial resources distribution for the Basic Care Plan for carrying out collective actions according to different health conditions. METHODS: A prior estimation of a global health index was performed by means of the analysis of major components prioritizing areas of Bogotá according to their health condition: "poor", "intermediate," and "good". A square minimum technique was applied to minimize difference between observed and expected global health index after resource investment. RESULTS: Resource distribution for the Basic Care Plan was achieved for twenty areas of Bogotá and a higher amount than the district median was allocated in areas with "poor" health conditions. Also, areas lacking universal health coverage were identified using the per capita resource allocation. CONCLUSIONS: This technique puts in evidence the disparity in health conditions between areas of "poor" health condition and those with "good" condition, in spite of the increased coverage of the Basic Plan of Attention, indicating the need of intersectorial social investment in these areas.

Progressive segmented health insurance: Colombian health reform and access to health services.

Equal access for poor populations to health services is a comprehensive objective for any health reform. The Colombian health reform addressed this issue through a segmented progressive social health insurance approach. The strategy was to assure universal coverage expanding the population covered through payroll linked insurance, and implementing a subsidized insurance program for the poorest populations, those not affiliated through formal employment. A prospective study was performed to follow-up health service utilization and out-of-pocket expenses using a cohort design. It was representative of four Colombian cities (Cendex Health Services Use and Expenditure Study, 2001). A four part econometric model was applied. The model related medical service utilization and medication with different socioeconomic, geographic, and risk associated variables. Results showed that subsidized health insurance improves health service utilization and reduces the financial burden for the poorest, as compared to those non-insured. Other social health insurance schemes preserved high utilization with variable out-of-pocket expenditures. Family and age conditions have significant effect on medical service utilization. Geographic variables play a significant role in hospital inpatient service utilization. Both, geographic and income variables also have significant impact on out-of-pocket expenses. Projected utilization rates and a simulation favor a dual policy for two-stage income segmented insurance to progress towards the universal insurance goal.

[Nursing education and the drugs phenomenon in Colombia: knowledge, attitudes and beliefs].

UNLABELLED: The objective of the research was to confer a distinction on knowledge, attitudes and beliefs in nurse formation, related with drugs problem. Descriptive study, Total sample 100% (n=159) from nursing pregrade Programs. RESULTS: The attitudes and beliefs Alcohol, Drugs, Alcohol and Drugs were known, and the knowledge that students have about drugs phenomenon. It identifies the relationship between International. CONCLUSION: The newly nurses Central Region of Colombia, theoretical preparation about Alcohol and Drugs, they assume positive attitudes, they show weakness in practice, to perform against this problem. They understand the relation between general health, international health and them with drugs problem, because all of this, it indicates they can be a great help in their international, nationals and regional contexts.
