Abstract

Recent improvements in intraocular lens (IOL) design and material delay posterior capsule opacification (PCO) after cataract surgery. However, studies have shown that without anterior capsule coverage of the IOL, PCO rates tend to increase. Surgeons have traditionally sized anterior capsulotomies without using a reference. The capsulotomy diameter mark is a convenient and inexpensive method to more consistently perform an anterior capsulorhexis that will allow anterior capsule overlap of the IOL optic.

Continuous curvilinear capsulorhexis with use of an endoscope.

We describe a technique for creating a continuous curvilinear capsulorhexis (CCC) using an endoscope. Three 2.0 mm limbal incisions 120 degrees apart were made in 4 cadaver eyes. The anterior capsule was stained with trypan blue 0.1% (Vision Blue). Under endoscopic visualization, a CCC was attempted in each of the 4 globes. It was completed in 1 of them. Endoscopy provides a safe and effective tool to visualize the anterior chamber and perform a CCC when there is a poor view through the cornea.

Inside-out delineation.

A technique of achieving precise hydrodelineation, in which fluid is injected from the inside of the nucleus to the outside, is described. The desired thickness of the nucleus-epinucleus bowl can be accomplished because the right-angled cannula can direct the fluid in the desired plane. This technique is useful in eyes with posterior polar cataracts and in eyes with dense cataracts in which aggressive phacoparameters are necessary for their removal.
