Accurate searching using XML and topic maps.

Abstract

Most clinical data is narrative text and often not accessible and searchable at the clinical workstation. We have therefore developed a search engine that allows indexing, searching and linking different kinds of data using web technologies. Text matching methods fail to represent implicit relationships between data, e.g. the relationship between HIV and AIDS. The ISO topic maps standard provides a data model that allows representing arbitrary relationships between resources. Such relationships form the basis for context sensitive searching and accurate search results. XML standards are used for the interchange of data relationships. Our approach has been applied to medical classification systems and clinical practice guidelines with promising results.

[Structural quality of neurologic rehabilitation clinics].

BACKGROUND: In 2003, criteria for assessing the structural quality of rehabilitation units for inpatient treatment were developed in cooperation with the German statutory pension insurance and health insurance. The aim of this study was to assess this quality using predefined valuation criteria and to test their applicability. METHODS: In the context of the Quality Assurance Programme for Rehabilitation provided by the German statutory health insurance, the structural quality of 18 neurological rehabilitation units was assessed applying the criteria for the first time. RESULTS: On average, the 18 rehabilitation units fulfilled 93.9% of the basic criteria. Despite this high proportion, variability in the degree of realisation in the individual rehabilitation units could be demonstrated. The basic criteria are realistic in respect to the structural requirements, without leveling out the differences between the centres. CONCLUSION: Assessing the structural quality of rehabilitation units on the basis of defined standards allows for a benchmark between units as well as for improvements within the individual units.

[Gustav Simon--Surgery of the kidneys].

In the English-speaking world anthologies about "classic works in urology" have been en vogue for years and have been published several times, but they do not exist in Germany. Due to the short half-life of knowledge, these pioneering publications were forgotten and are not readily available or easily accessible. Furthermore, for reasons of preservation libraries do not allow borrowing these old books or copying them. These articles thus remain inaccessible to the public. The work of the famous Gustav Simon ranks along with Nitze's "Kystoscopy" among the most important works for our specialty. The Museum and Archives of the German Urologic Society, located at the head office in Dusseldorf, has a well-renowned collection of papers, monographs, and books from across the centuries.

[Survey of incidents associated with hip and knee replacement devices. Analysis of the surveillance and registration system for medical products for the years 2000-2002].

BACKGROUND: The purpose of this study was to address the experience gained from the risk assessment of incidents associated with hip and knee joint replacement devices. METHODS: A total of 274 incidents were reported throughout the years 2000-2002. A retrospective survey of all reports was conducted. RESULTS: Of all cases, 85.0% were associated with reported patient injury (n=274). In 88.4% of these cases injury consisted in revision surgery (n=233). Focusing on the cases alone where implant lifetime was known, 42.6% of these incidents occurred within the 1st year and 87.9% within 5 years (n=148). Only 20.4% of all incidents were caused by a production error (n=274); 36.2% of all incidents directly led to corrective action (n=274). In 10.2% the incident occurred at a time when corrective action had already been taken (n=274); this corresponds to 23.0% of all patients involved in incidents (n=291). CONCLUSION: In general, improvements in implant design during the past decade have increased the level of protection of health and safety of hip and knee joint replacement devices. Nevertheless, our data suggest that there is still a certain amount of design problems contributing to implant failure. By increasing the number of reports, users could help to enhance the safety of these devices.
