[Chronified headache].

Abstract

Paroxysmal headache of the migraine type as well as permanent undulating headache (which we call cephalea) can lead to chronification, both often mixes within the chronification. Existence of psychogenic factors (in the broader sense) favourises chronification of headache. A self supporting circulus vitiosus may arise, in combination with the cervical column, depressivity, whiplash-injury, chronical over-use of drugs (often not without an iatrogenic component), expertise situations. Therapeutically we emphasise a polypragmasy orientated on target-symptoms. Analgesics and tranquilizers want to be excluded as much as possible. Clearly indicated antidepressant are of great value, physiotherapy and psychotherapy as well. As ultima ratio we administer a neuroleptic sleeping cure.

[Meningeal melanocytoma or multiple pigmented meningioma of the spinal canal. Report of a case. Review of the literature].

In a 64-year-old male with pain and weakness of the right lower limb, investigations disclosed a cervical tumor, several dorsal tumors, and tumoral infiltration of the lumbosacral area. Histological and, above all, immunohistochemical and ultrastructural features were suggestive of meningeal melanocytoma. Before the use of immunohistochemical and electron microscopy techniques, meningeal melanocytoma was designated by the term pigmented meningioma because its histological features bear some resemblance to those of meningioma. However, only a few cases of meningeal melanocytoma have been documented by immunohistochemistry and electron microscopy. Previously reported patients with meningeal melanocytoma had solitary tumors. In contrast, a review of the literature showed that multiple intraspinal or intraspinal and intracranial meningiomas are not exceptional, although meningiomatous infiltration is considerably less frequent. Treatment of such cases rests on surgery, radiotherapy, and chemoimmunotherapy.

[Importance of prognostic factors in the prognosis of cancer of the endometrium].

126 stage I and II endometrial cancer patients were followed up for at least 5 years to evaluate the impact of surgical-pathological risk factors on the survival. Out of the prognostic variables evaluated the age and clinical stage failed to influence the 5-year survival rate. The effect on survival of factors such as low grade of differentiation and the depth of myometrial infiltration was however significant, (p < 0.01 and p < 0.001 respectively). The early local recurrence has also significant negative prognostic value (p < 0.05). This latter can be effectively prevented through the prehysterectomy suture of the external cervical os. On the basis of their findings authors suggest to consider the grade and the depth of myometrial invasion beside the clinical stage at the determination of the extension of the surgical procedure and at the decision on the administration of postoperative irradiation.
