Parallel bonded cast attachments for removable partial dentures.

Abstract

The original design of a semi-precision extra-coronal attachment for partial dentures is illustrated, and its fabrication in a cast non-precious bondable alloy is described. A step-by-step method is also described of bonding multiple attachments in parallel for the retention and support of removable partial dentures. Clinical cases are illustrated. The results of trials indicate that the method is clinically viable and, in selected cases, appears to offer distinct advantages over existing retention systems.

Incidence of various classes of removable partial dentures.

The purpose of a classification for removable partial dentures (RPDs) is to simplify identification and enhance its teaching. A classification also allows a longitudinal comparison of various classes of RPDs to determine whether the teaching of RPD design is consistent with the relative frequencies of RPD use. This study surveyed the types of removable partial dentures being fabricated in a regional dental laboratory and compared these findings with data from previous studies. Results indicate that mandibular RPDs are more common than maxillary RPDs and the class I mandibular RPD is the most common type of RPD for either dental arch. A palatal strap was the most frequently used maxillary major connector and a lingual bar was used three times more often than a lingual plate in the mandibular arch. Cast circumferential clasps were used twice as often as RPI clasp designs. The percentage of Kennedy class I RPDs was 40%, class II 33%, class III 18%, and class IV 9%. Comparisons with a previous study indicate the percentage of Kennedy class II has increased, whereas class I, class III and class IV RPDs have not changed significantly. Findings of the study indicate that the frequency of use of the various types of RPDs have changed in the past 30 years.

Bar-retained overdentures for implants--technical aspects.

The principles of the laboratory procedures for making overdentures retained by bar abutments on ITI implant fixtures are described, and details of producing the master casts, making a soldered bar, and processing a prosthesis incorporating a cobalt-chromium alloy strengthener are given.

Use of plastic bar molded casting in the modification of variably expanding investment to compensate for casting shrinkage of nickel-chromium alloy.

Trial plastic bar molded castings were compared for accuracy with cast crowns by means of a nickel-chromium alloy, which was used to establish the liquid density of variably expanding investments. The plastic bars were invested to evaluate the change of expansion rate in a wide range of six liquid densities from 0% to 100%, and the distances between the sections were measured before and after casting. Wax crowns were cast to obtain a more detailed relation around 0% expansion at seven liquid densities, and the gaps between the base of the die and the margin of the crown were measured before and after casting. The expansion rates of both castings were calculated and thus were proportional to the liquid density; both regression curves indicated high correlation coefficients. As a result of the statistics of the Student's t-test, the difference between the two methods was not significant. The trial plastic bar was useful in establishing the variably expanding investment for precise casting of artificial crowns.
