Abstract

Reductionism has served cell biologists well in the past decades, but systems biologists are now promising to broaden the picture. The challenge will be to provide not just the lists and quantitation but real mechanistic insights.

[Relevance of surgical outpatient clinics in academic university centers to health care in Germany].

During the observation period between 2001 and 2003, all outpatient surgical therapy, including degrees of urgency, surgical care volume, regional provenance of patients, diagnoses, and referral channels were prospectively analysed at the Surgical Department of the University of Heidelberg, Germany. The data gathered do not merely describe the volume and characteristics of care encountered at this academic surgical institution but also provide further insight into the variability of resource utilisation and associated patient flow. Additionally, a retrospective evaluation using structured interviews and questionnaires was performed to differentiate and quantify patient care, teaching, and research activities. This study illustrates the high relevance of academic outpatient institutions to regional provision of general surgical care in Germany. There is a clear dominance of medical support functions, while research and teaching activities are of only minor relevance and realised particularly in subspecialty clinics. These data should give important stimuli for the future planning of health care in Germany. Outpatient clinics for general surgery appear to be an excellent basis for regional models of integrated health care delivery in the future.

Diffusion of breast conserving surgery in medical communities.

Excluding skin cancers, breast cancer is the most common form of cancer in women. Due to an increased focus on early detection, many more cases of breast cancer are now diagnosed at an early stage, which makes the use of breast conserving surgery (BCS) an efficacious and often more desirable treatment choice than mastectomy. An analysis of the variation in the use of BCS in the United States was performed using data from the years 1988 and 1994, and stratifying hospitals on the basis of teaching status. In both 1988 and 1994, BCS was highest in academic teaching hospitals and lowest in community hospitals. This finding is interpreted within the framework of classical diffusion theory. Social and cultural norms in local medical communities have a strong effect on the degree to which innovations diffuse rapidly or not. This analysis is useful in the understanding of geographic and hospital-based variations in treatment for early stage breast cancer and other illnesses that have long and strongly held traditions of treatment.

The complexities of individual financial conflicts of interest.

Academic health centers (AHCs) are unique resources that are critical for the advancement of science and the training of health care providers, scientists, and educators. AHCs depend on public trust. Certain financial relationships between medical school faculty and industry create situations that have the potential to be deleterious to AHCs. Yet, the translation of clinically relevant, scientific advances to products that directly impact patients' lives benefits from entrepreneurial activities, and such activities necessitate interactions between academia and industry. Society has a vested interest in protecting human research participants, maintaining objectivity in scientific research, and encouraging creative research with clinical applications. Conflicts of interest (COI) committees have been created by AHCs to evaluate faculty financial COI and to develop strategies to eliminate, reduce, or manage such conflicts. Issues involving financial COI are relevant to psychiatry. These issues are reviewed from the perspective of regulatory oversight provided by a medical school's COI committee.
