[Flunitrazepam and driving ability].

Abstract

Major contributions to the investigation of the influence of drugs on driving ability come from experimental studies as apart from epidemiological studies. Regarding experimental studies, the results of a single study are essentially determined by the experimental design. An established assessment of the risk of a drug in regard to driving ability should not be based on either a single study or on synopses, which record only a few studies without detailed information. This article presents an overview of published studies that directly or indirectly deal with the influence of flunitrazepam on driving ability. Benzodiazepines in particular, and amongst these especially flunitrazepam are brought into connection with the impairment of driving ability. This approach emphasises differences regarding the dosages used, the experimental designs and the methods of testing. For reasons of clarity the article on flunitrazepam is subdivided into the topics of acute effects, residual effects, and effects after subchronic application. Different results are reported. A statistical analysis in terms of a meta-analysis was not carried out due to a lack of standardization of the studies.

Bayesian predictive approach to interim monitoring in clinical trials.

This paper reviews Bayesian strategies for monitoring clinical trial data. It focuses on a Bayesian stochastic curtailment method based on the predictive probability of observing a clinically significant outcome at the scheduled end of the study given the observed data. The proposed method is applied to derive efficacy and futility stopping rules in clinical trials with continuous, normally distributed and binary endpoints. The sensitivity of the resulting stopping rules to the choice of prior distributions is examined and guidelines for choosing a prior distribution of the treatment effect are discussed. The Bayesian predictive approach is compared to the frequentist (conditional power) and mixed Bayesian-frequentist (predictive power) approaches. The interim monitoring strategies discussed in the paper are illustrated using examples from a small proof-of-concept study and a large mortality trial.

A functional continuum of regulatory anxiety-enhancing peptides. The search for complexes providing the optimal basis for developing inhibitory therapeutic agents.

Regulatory peptides are actively involved in controlling most physiological processes. One such function is regulation of the level of anxiety and panic states. We report here a meta-analysis of data published from 1960 to 2004 on the effects of anxiety-enhancing regulatory peptides. The resulting database was used to investigate the organization and functioning of the anxiogenic regulatory peptide system. Using vector representation of the effects of these peptides, the spectra of physiological effects which might be provoked by each anxiety- and fear-increasing regulatory peptide alone and in combination were evaluated. Complexes of regulatory peptides with anxiogenic profiles with the greatest potential for the further experimental development of inhibitory pharmacological agents were identified.

Psychopharmacology interventions for pediatric anxiety disorders: a research update.

Pediatric anxiety disorders are commonly occurring mental health disorders of childhood and are associated with significant distress, impairment, and risk for ongoing morbidity. During the past decade, significant progress has been made in developing safe and effective therapeutic approaches to these disorders. This article focuses on the scientific literature supporting the use of a variety of medication approaches to the treatment of the various anxiety disorders of childhood and adolescence.
