Abstract

A critical review of techniques used in the assessment of microleakage in dental restorations is presented. These techniques include the use of air pressure, bacteria, radioisotopes, electrochemistry, chemical tracers and dye penetration. Wide variations in methodologies are revealed.

In vitro evaluation of dynamic fluid displacement in dentinal tubules activated on pin placement.

The use of cemented, friction-lock, and self-threading pins for improving retention has been essential for treatment in restorative dentistry, with the self-threading pin considered to be the most retentive. Cavity varnish has been suggested to prevent microleakage around pins. This study investigated the insertion of a self-threading pin when the pinhole was filled with a liquid dye. The results showed that pinholes filled with dye before pin placement had measurable dye displacement. Pinholes without pins displayed no measurable dye displacement through the dentin; this was also true when the dye fluid was removed before the pin placement. However, the dye in pinholes was displaced toward the path of least resistance, and fluid in a pinhole can contribute to crazing of the dentin during placement.

Sealing ability of Thermafil with and without sealer.

The purpose of this study was to evaluate the apical sealing ability of root canals filled using three obturating techniques. Sixteen maxillary first molars were obturated with Thermafil, 16 with Thermafil and a ZOE sealer, and 16 with laterally condensed gutta-percha. Two canal instrumentation methods were used, conventional step-back preparation with K-Flex files and traditional instrumentation combining reaming with reamers and filing with K-Flex files. Following obturation, the teeth were prepared for evaluation of the seal using India ink and a Profile Projector. The depth of penetration of the dye was statistically evaluated for each of the three roots with an analysis of variance. For sealing ability, there was no significant difference at the p < 0.05 level between the conventional step-back preparation and the traditional instrumentation technique. However, a significant difference was present at the p < 0.05 level for the obturation techniques. The mean linear dye penetration for the Thermafil technique was greater than that for lateral condensation.

Microleakage comparison of apical seal of plastic versus metal Thermafil root canal obturators.

This study compared the apical seal of plastic versus metal Thermafil obturators in extracted human teeth. The obturated teeth were evaluated radiographically for the extent and adaptation of the filling obturator. No difference was seen between plastic and metal for over- or underfilling. Voids were seen more often in smaller sizes of both plastic and metal obturators. Obturated teeth were stained in India ink, decalcified, and cleared. Microleakage evaluation of the transparent teeth showed no statistical difference between metal and plastic obturators.
