Evidence-based practice: a practical approach to implementation.

Abstract

Organizations often do not have processes in place to support nurses through a systematic approach for developing and evaluating nursing interventions, protocols, critical pathways, and policies that are derived from scientific evidence. The development of a framework to guide inquiry will have a positive impact on patients. This process may foster a higher level of professional engagement by nurses that may, in the long-term, help improve nurse retention and recruitment. The authors discuss a nursing evidence-based practice model and guidelines that were developed by a team of hospital and academic nurse leaders and is practical and easy to use. This model has been successfully implemented across the department of nursing as a strategic initiative. Results of the implementation have shown that staff nurses can effectively use this model with the help of knowledgeable mentors.

Episiotomy: beliefs, practice and the impact of educational intervention.

OBJECTIVE: To assess the potential effect of educational intervention on episiotomy practice. STUDY DESIGN: A survey of the attitude of obstetrical caregivers toward episiotomy was conducted among obstetricians and midwives in the three public hospitals in Haifa. Data regarding episiotomy rates was collected for the years 2001-2003. At the beginning of 2002, lectures on the risks and benefits of episiotomy were given in two hospitals. Episiotomy rates before and after the lecture were compared. RESULTS: A significant and lasting reduction in episiotomy rates was observed in the two hospitals where lectures were given. There were no clinically significant and consistent changes in the episiotomy practices in the third hospital. CONCLUSION: Education may play an important role in changing common medical practices, as in episiotomy. It was clearly shown that our beliefs are not always up to date. We call for periodic reassessment of all medical procedures, as common and accepted as they are.

Oncologists' opinions towards recommending exercise to patients with cancer: a Canadian national survey.

RATIONALE: The goal of this study was to examine oncologists' attitudes towards recommending exercise to patients with cancer during treatment. PATIENTS AND METHODS: Using a national survey, all 659 practicing medical and radiation oncologists in Canada were sent a brief questionnaire. RESULTS: The response rate was 46% (281/610). The majority of oncologists agreed that exercise was beneficial (62.0%), important (55.8%) and safe (63.1%) for patients with cancer during treatment. Forty-three percent of oncologists reported that they tried to recommend exercise to their patients when appropriate. Moreover, oncologists actually reported recommending exercise to 28% of their patients during the past month. Analyses also indicated significant differences between oncologists, with younger, female, and medical oncologists generally having more favorable attitudes towards exercise for patients with cancer than their older, male, and radiation oncology counterparts. CONCLUSIONS: Oncologists have a favorable attitude toward recommending exercise to patients with cancer although several important barriers may prevent oncologists from providing exercise advice to their patients. Further research is required evaluating the effectiveness of interventions and strategies designed to improve oncologists' confidence and ability to advise their patients on exercise during oncology consultations.

Is success in postgraduate examinations associated with family practitioners' attitudes or patient perceptions of the quality of their consultations? A cross-sectional study of the MRCGP examination in Great Britain.

BACKGROUND: Postgraduate examinations are ubiquitous in medicine worldwide, but studies to validate them are rare. The Royal College of General Practitioners of the UK, over the years in an evolving format, has offered a membership examination (MRCGP) which it believes acts as a quality marker for those who sit it and also positively influences the development of family practice generally. It is not clear, however, if this process identifies quality markers that patients can perceive. OBJECTIVES: To determine if possession of the MRCGP (a doctor defined measure of doctor quality) is associated with the patient enablement score (a patient based consultation outcome measure) and family practitioners' attitudes to the work of family practice. METHODS DESIGN: survey using the Patient Enablement Instrument (PEI) with linked survey data on family practitioner (FP) demography and possession of the MRCGP, and FPs' attitudes and beliefs using the Cockburn attitudinal questionnaire. SUBJECTS: 15 534 adult patients attending 154 FP principals. SETTING: 50 family practices in the UK. OUTCOME MEASURES: the association between possession of MRCGP, and PEI and Cockburn scores was assessed using regression analysis controlling for known confounders. RESULTS: There was no association between PEI score and possession of the MRCGP. Only one scale of the Cockburn attitude questionnaire (the belief that patients should be involved in decision making) was positively associated with possessing the MRCGP. CONCLUSION: Any advantage in physician quality conferred by passing the MRCGP exam was not detected in this study. Further research into the predictive validity of postgraduate examinations is required preferably using a wider variety of patient and audit based methods.
