[Empathy].

Abstract

Empathy has been written about for over a Century. It occurs as a non-judgemental clinician-patient relationship with powerful effects. The clinician develops particularly careful listening and observation skills using his "body analyser". He indicates to his patient that he understood his affects and accepts them. The patient feeling understood would generally suffer less, comply better with the treatment, which will have a better outcome. However, empathy isn't always easy to feel, but it can be learned and improved.

Defining the competencies of those conducting pharmacovigilance.

PURPOSE: The absence of agreed competencies in pharmacovigilance and patient risk management is a key factor hindering the development of training and a curriculum in these interrelated disciplines. If competencies were to be developed, they could be tailored to the different roles and responsibilities of the various staff involved. PROPOSAL: We describe a simple theoretical model of functional and behavioural competencies that is based on an organisation with three levels of staff: evidence collectors and gatherers, evidence processors and distillers, and decision makers. CONCLUSIONS: To stimulate discussion about how to enhance the progress already being made in pharmacovigilance and patient risk management, we have proposed functional and behavioural competencies for pharmacovigilance as a way of identifying and measuring scientific and personal skills pertinent to pharmacovigilance. Competencies should, we believe, be agreed and adopted by all those seeking to improve pharmacovigilance. We invite discussion of these proposals.

[The concept of disease in psychiatry].

To draw the line between health and disease has considerable consequences for medical action and financing of medical services as well. It will be shown that satisfying scientific reasons cannot be given for a general and practicable definition either of health or of disease. This is possible only for a specific concept of disease that can be assessed by a diagnosis. However, even such a specific term may be ambiguous, particularly in psychiatry, for which it is of central importance how and what the patient communicates of his experiences. The reason lies in different ways of assessing: the physician describes phenomena of the patient that are controllable by standardisation, the patient ascribes his own experience uncontrollably individually. Moreover, the moment of appeal in the ascriptive mode indicates a call from the specific need for the help of others, of the community. The descriptive mode is relevant for the generalisable assessment of a disease, the ascriptive mode for the assessment of the individual illness.
