Intranasal zolmitriptan.

Abstract

Intranasal zolmitriptan has recently been launched in the US and Canada and is a novel formulation of the antimigraine drug zolmitriptan. This drug has been used extensively in its oral formulation. The intranasal formulation offers a more rapid onset of action. Pharmacokinetic studies have demonstrated that it is absorbed rapidly into the systemic circulation and traces can be seen in the brain within 5 min. The onset of efficacy in the Real Life Intranasal Zolmitriptan Exposure (REALIZE) study was measurable at 10 min and at 15 min in the US 22 study, which demonstrates rapid absorption and effect at the 5-HT(1B/1D) receptors. This article reviews the initial studies of intranasal zolmitriptan; three of the most recent studies will be examined in detail.

Comparing two crossing hazard rates by Cox proportional hazards modelling.

Motivated by a clinical trial of zinc nasal spray for the treatment of the common cold, we consider the problem of comparing two crossing hazard rates. A comprehensive review of the existing methods for dealing with the crossing hazard rates problem is provided. A new method, based on modelling the crossing hazard rates, is proposed and implemented under the Cox proportional hazards framework. The main advantage of the proposed method is the utilization of the Box-Cox transformation which covers a wide range of hazard crossing patterns. Simulation studies are conducted for comparing the performance of the existing methods and the proposed one, which show that the proposed method outperforms some of its peers in certain cases. Applications to a kidney dialysis patients data and the zinc nasal spray clinical trial data are discussed.

Response of calves to challenge exposure with virulent bovine respiratory syncytial virus following intranasal administration of vaccines formulated for parenteral administration.

OBJECTIVE: To determine whether single-fraction and combination modified-live bovine respiratory syncytial virus (BRSV) vaccines commercially licensed for parenteral administration could stimulate protective immunity in calves after intranasal administration. DESIGN: Randomized controlled trial. ANIMALS: 39 calves. PROCEDURES: Calves were separated from dams at birth, fed colostrum with a minimal concentration of antibodies against BRSV, and maintained in isolation. In 2 preliminary experiments, 9-week-old calves received 1 (n = 3) or 2 (3) doses of a single-component, modified-live BRSV vaccine or no vaccine (8 control calves in each experiment), and were challenged with BRSV 21 days after vaccination. In a third experiment, 2-week-old calves received combination modified-live virus (MLV) vaccines with or without BRSV and calves were challenged with BRSV 8 days later. Calves were euthanized, and lung lesions were measured. Immune responses, including serum and nasal antibody and nasal interferon-alpha concentrations, were assessed. RESULTS: BRSV challenge induced signs of severe clinical respiratory tract disease, including death and pulmonary lesions in unvaccinated calves and in calves that received a combination viral vaccine without BRSV. Pulmonary lesions were significantly less severe in BRSV-challenged calves that received single or combination BRSV vaccines. The proportion of calves that shed virus and the peak virus titer was decreased, compared with control calves. Protection was associated with mucosal IgA antibody responses after challenge. CONCLUSIONS AND CLINICAL RELEVANCE: Single and combination BRSV vaccines administered intranasally provided clinical protection and sparing of pulmonary tissue similar to that detected in response to parenteral delivery of combination MLV and inactivated BRSV vaccines previously assessed in the same challenge model.

Efficient vagina-to-lower respiratory tract immune trafficking in a murine model of influenza A virus infection.

Effective vaccination strategies for infectious diseases take into account the induction, long-term maintenance and recall of memory T-cell populations. To understand the immunological cross-talk within the mucosal compartments, we compared intranasal to vaginal immunization and demonstrated that vaginal infection of BALB/c mice with influenza A virus provides protective mucosal immunity against both homosubtypic and heterosubtypic virus challenge in the respiratory tract. We found that, prior to the viral challenge, in vaginally primed mice, antigen-specific CD8+ T cells were not detected in the lung airways and levels of serum antibodies were lower than those observed in intranasally immunized mice. However, following pulmonary challenge, NP147-specific CD8+ T cells were recruited and amplified in vaginally primed mice to the same extent as those in intranasally primed mice. Thus, the long-term memory immune response elicited by vaginal immunization with influenza virus is efficiently recalled and offers reasonable protection against infection in the respiratory tract.
