Mechanisms of regional wall motion abnormalities in contrast-enhanced dobutamine stress echocardiography.

BACKGROUND: In the diagnosis of coronary artery disease (CAD) with Dobutamine Stress Echocardiography (DSE), regional wall motion abnormalities (RWMA) are assumed to indicate a perfusion deficit. METHODS AND RESULTS: For a more particular examination of RWMAs, we compared simultaneous echo-contrast (Optisone)-enhanced DSE (0-40 microg/kg Dobutamine, 16-segment- model) and MiBi-SPECT in a prospective double-blinded study design in 69 non-selected consecutive patients (44 male, 25 female, age 64+/-12 years). Additionally, all patients were examined by coronary-angiography. The prevalence of significant CAD (stenosis >50% lumen diameter) was 52%. DSE had a sensitivity of 78% and a specificity of 66% for the detection of significant CAD with a positive and negative predictive value of 72 and 73%, respectively. Among 28 patients with significant CAD and positive DSE study (true positive), 78% displayed a corresponding perfusion deficit in MiBi-SPECT. Among 11 patients with a positive DSE study but no current significant coronary stenosis (false positive), 82% showed stress-induced RWMAs in the inferior/posterior region, 73% displayed left ventricular hypertrophy, 54% resting-ECG abnormalities and 45% resting-RWMA (3 previous MI, 2 previous CABG surgery). Among 8 patients with negative DSE study but significant coronary stenosis (false negative), 75% had a stenosis of the LCX, 63% displayed resting- WMA, 63% displayed left bundle branch block or ST-segment depression, 50% displayed only peripheral coronary stenosis, and DSE visualization was suboptimal in 38%. CONCLUSION: This prospective study in non-selected patients shows that the majority of RWMAs in DSE are matched to a perfusion deficit detectable by nuclear imaging. Nevertheless, pre-existing cardiac abnormalities may also lead to stress-induced RWMA not associated with a perfusion deficit or mask a perfusion deficit upon DSE. Particularly in patients with LV hypertrophy, resting-RWMA, bundle branch block or ST segment depression, the predictive value of DSE may, therefore, be limited.

Effect of cyclodextrin nanocavity confinement on the photorelaxation of the cardiotonic drug milrinone.

We report on steady-state UV-visible absorption, emission, and picosecond emission studies of milrinone (MIR) drug in neutral water and complexed to cyclodextrins (alpha-, beta-, gamma-CD and dimethyl-beta-CD (DM-beta-CD)). The results reveal that MIR forms a 1:1 inclusion complex with CD. Upon encapsulation the emission intensity increases and the fluorescence lifetime changes from approximately 65 ps to 240-350 ps, indicating a confinement effect of the nanocages on the photophysical behavior of the drug. Due to its methyl groups, the DM-beta-CD complex shows the largest effect. The time-anisotropy experiments support the formation of 1:1 inclusion complexes and indicate motion of the drug inside the nanocavity. Furthermore, results of PM3 calculations combined with spectral and dynamical data show that the drug is not fully embedded into the cavities, and the conformation of the included complex explains the relatively short lifetimes and low emission quantum yields of these entities.

Safety of stress echocardiography (from the International Stress Echo Complication Registry).

The safety of any diagnostic test is a major issue in deciding its practicability and cost-effectiveness. The aim of this study was to evaluate the safety of various stress echocardiographic modalities in the "real world." From February 1998 to January 2004, a simple written questionnaire was distributed to echocardiography laboratories across the world known to perform stress echocardiography. The following categories of major complications (known to occur during stress testing) were indicated: sustained ventricular tachycardia (> 30 beats/min), ventricular fibrillation, myocardial infarction, third-degree atrioventricular block, severe hypotension requiring therapy, cardiac asystole, cardiac rupture, stroke, and death. Three hundred centers were polled, from which 71 co-investigators responded and reported on 85,997 patient examinations. Exercise was used in 26,295 cases, dobutamine in 35,103, and dipyridamole in 24,599 cases. Life-threatening events occurred in 86 cases: during exercise in 4 patients (event rate 1 in 6,574), during dobutamine infusion (small dose for viability and/or large dose for ischemia) in 63 patients (event rate 1 in 557), and during dipyridamole stress testing in 19 patients (event rate 1 in 1,294). Of the 86 patients with complications, 5 died during dobutamine stress testing (ventricular fibrillation, n = 2; cardiac rupture, n = 3) and 1 after dipyridamole testing (cardiogenic shock). In conclusion, stress echocardiography is a safe method in the real world, but serious complications may occur. Exercise seems safer than pharmacologic stress and dipyridamole safer than dobutamine, possibly because of preselection criteria.
