[Visual dependence after recent stroke].

Abstract

INTRODUCTION: After chronic stroke, inability to use pertinent somatosensory or vestibular information have been described. The aim of the study was to determine whether visual dependence occurred early after stroke before rehabilitation. METHOD: Thirty patients with recent hemiplegia (16 right and 14 left hemispheric stroke) performed the rod and frame test (RFT). Patients were asked to adjust the rod to the vertical position under 3 conditions: basically, with a frame tilted 18 degrees to the right and then with the frame tilted to the left. Bias in each condition (mean, SD) was recorded and compared to adjustments of the rod by 23 controls. Motor control, sensibility, functional level (functional independence measure), age, neglect, and then balance by the postural assessment scale for stroke were assessed. RESULTS: Fifty-six per cent (17/30) of patients but only 26% of controls were influenced by the tilt of the frame on the 2 sides (visual dependence). No correlation was found between visual dependence and the characteristics of the patients. DISCUSSION: Many patients with recent hemiplegia seem to rely on visual input. The mechanisms of such visual dependence are discussed. Rehabilitation programs should take into account the possible impairment of sensory organisation and should include exercises to be performed under visual disturbances.

Adaptive design method based on sum of p-values.

Bauer and Kohne proposed an adaptive design using Fisher's combination of independent p-values based on subsamples from different stages (Biometrics 1994; 50(4):1029-1041). Their method provides great flexibility in the selection of statistical methods for hypothesis testing of subsamples. However, the choices for the stopping boundaries are not flexible enough to meet practical needs (Biometrics 2001; 57(3): 886-891). In this paper, an adaptive design method is proposed using linear combination of the independent p-values. The method provides great flexibility in the selection of stopping boundaries and no numerical integration is required for the two-stage designs. The stopping boundaries and p-values can be calculated manually. The operating characteristics of the adaptive designs are studied using computer simulations with and without sample size adjustment. Examples are presented for superiority and non-inferiority trials with different endpoints (normal, binary, and survival) under different adaptations. The statistical efficiency of the proposed method is compared with other methods based on conditional power.

A nonlinear latent class model for joint analysis of multivariate longitudinal data and a binary outcome.

We consider a joint model for exploring association between several correlated longitudinal markers and a clinical event. A nonlinear growth mixture model exhibits the different latent classes of evolution of the latent quantity underlying the correlated longitudinal markers and a logistic regression models the probability of occurence of the clinical event according to the latent classes. By introducing a flexible nonlinear transformation including parameters to be estimated between each marker and the latent process, the model also deals with non-Gaussian continuous markers. Through an application on cognitive ageing, the two advantages of the model are underlined: (1) the latent profiles of evolution associated with the clinical event are described including covariate effects in the longitudinal model but also in the probability of class membership and in the probability of occurence of the event, and (2) a diagnostic and a prognostic tools are derived from the model for early detection of the clinical event using any available information about the longitudinal markers.
