[The problems in anesthetic support in developing countries].

Abstract

We report our support for anesthesia in Afghanistan and northern Iraq. The anesthetic staff of both countries expressed their wish to learn regional anesthesia. In Afghanistan, anesthesia is carried out only by nurse anesthetists, who need basic education in anesthesia. Due to the shortage of medical supply, the present condition is not suitable for regional anesthesia training. In northern Iraq, there are adequate medical supply and competent anesthetists, and the training of regional anesthesia was possible. The sustainability of health care depends on its country's economic power. The technical support as an aid is difficult, if the level of support exceeds the capacity of the economic power.

[Charcoal smoke causes bronchial anthracosis and COPD].

BACKGROUND: Bronchopulmonary disease due to inhalation of smoke from open woodfires represents a major health problem in developing countries. Due to increasing migration such patients also present to medical services in Europe. CASE REPORT AND DISCUSSION: An 84-year-old Afghan housewife who never smoked nor has a history of exposure to inorganic dusts, presents with chronic obstructive pulmonary disease (COPD) in association with bronchial anthracosis and stenosis of a bronchus. The complaints are found to be caused by chronic inhalation of smoke from an open woodfire which was used for cooking. The main complaints of "woodsmoke-associated lung disease" are cough und dyspnea with bronchial obstruction. Radiology and bronchoscopy usually reveal changes which are similar to pneumoconiosis of miners but without patients' relevant exposure. There is a frequent association of anthracotic bronchial stenosis and infection with tuberculosis. CONCLUSION: Since patients rarely recognize the risks of woodsmoke inhalation, they hardly report their exposure. Thus, the anamnesis is crucial to establish the right diagnosis and guide the patient to the appropriate diagnostic and therapeutic procedures.

Place, health and home: gender and migration in the constitution of healthy space.

This paper contributes to recent literature that considers the role of everyday activity in constructing 'healthy space', specifically exploring the tension between agency and structural processes in explanation. The focus is a comparison of two groups of migrant women in British Columbia, Canada: South Asian Sikhs from Punjab, India, and Afghan-Muslim refugees. It explores the routine practices whereby they work to create 'healthy space' as they orchestrate their families' health. Through food preparation and consumption practices, traditional healing and religious observance, the women delineate the physical, social and symbolic dimensions of healthy space. The women's narratives demonstrate the productive capacity of everyday routines in forging healthy space within the particularities of migrant settlement.
