Nutritional support in liver failure.

Abstract

The most important manifestations of advanced liver disease are malnutrition, encephalopathy, and hepatorenal syndrome. Appropriate and optimal nutritional and metabolic support with specially formulated solutions enriched in branched-chain amino acids, together with other advanced treatments such as plasmapheresis, hemofiltration, and continuous arteriovenous hemodialysis, can correct the most important metabolic and clinical aberrations and temporarily improve the quality of life. However, the ultimately poor prognosis of patients with severe liver failure has not been altered significantly to date by the provision of nonspecific or specifically tailored nutrient regimens. The prognosis appears to depend on the arrest, correction, or reversal of the primary pathophysiologic process.

Nutritional modulation of the inflammatory response in inflammatory bowel disease--from the molecular to the integrative to the clinical.

Nutrient deficiencies are common in patients with inflammatory bowel disease (IBD). Both total parenteral and enteral nutrition provide important supportive therapy for IBD patients, but in adults these are not useful for primary therapy. Dietary intervention with omega-3 polyunsaturated fatty acids contained in fish oil may be useful for the care of IBD patients, and recent studies have stressed the role of PPAR on NFkappaB activity on the potential beneficial effect of dietary lipids on intestinal function.

Enteral water for hypernatremia and intestinal morbidity in infants less than or equal to 1000 g birth weight.

OBJECTIVE: To evaluate the relationship between enteral water infusion for hypernatremia and significant intestinal morbidity in infants or=150 (high sodium control), >or=150 and treated with sterile water (study group). Significant intestinal morbidity was defined as probable or proven necrotizing enterocolitis or spontaneous intestinal perforation. Statistical analysis included Student's t test for continuous variables and chi(2) with Yeats correction for frequency variables. Multivariate logistic regression analysis was then performed to evaluate confounding variables among groups. RESULTS: The incidence of intestinal morbidity was significantly higher in the high sodium-water treated group compared to each of the other groups (13/33 (38%) for high sodium-water versus 16/100 (16%) for high sodium control and 18/188 (10%) for normal sodium control, P<0.01 chi(2)). Logistic regression analysis indicated that enteral water and hydrocortisone were risk factors for significant intestinal morbidity. CONCLUSIONS: Enteral sterile water for hypernatremia appears to be associated with significant intestinal morbidity in infants <or=1000 g. Hydrocortisone is also a risk factor.
