[Analytical morphodynamics. Practical applications].

Abstract

The analytical morphodynamic method, based on the study of the fate of the biogic units of an organ at the level of well or poorly developed lesions, has been applied to the analysis of chronic obliterating arteriopathies of the extremities, of benign tumors of the mammary parenchyma, and of malignant hepatomas. In morbid states as different as a reactive process and a benign or malignant neoplastic process, it allowed to reconstritute the histogenesis of complex tissue changes and led to interesting prospects, in the field of general biology and pathogenesis.

[Buchwald's operation in the surgery of atherosclerosis obliterans].

Analyzing the world's experience in the surgical management of hyperlipidemias and their own results, the authors make an assessment of Buchwald's operation used in the surgery for atherosclerosis obliterans. The major disadvantage of partial ileac bypass is that there is no prognosis of the operation and its complication. Based on the pathophysiological mechanisms of implementation of Buchwald's operation, the authors have developed a mode of pharmacological simulation of a partial bypass which allows one to predict the effect of the impending intervention and to avoid unjustified operations. The unique procedure also enables the indications for simultaneous interventions to be defined. Ten partial ileac bypass surgeries made by the authors are comprehensively analyzed. Taking into account, the authors propose to choose a treatment of patients with hyperlipidemia and atherosclerosis on an individual basis and to do away with the routine indications for partial ileac bypass surgery.

Arteriosclerosis Obliterans

Hematologic changes in type 2 diabetic patients with various localizations of peripheral vascular disease.

Some hematologic parameters have been measured in 80 patients with Type 2 diabetes mellitus. The patients were divided into three groups: (a) proximal (pelvic and femoral) lower limb macroangiopathy; (b) distal (tibial) macroangiopathy; and (c) control group of diabetics without lower limb macroangiopathy. Mean hemoglobin, hematocrit and fibrinogen values were significantly higher in both the proximal and distal macroangiopathy groups in comparison with the controls. On the other hand, mean leucocyte and platelet counts were significantly elevated only in the proximal macroangiopathy group. Significant correlations were observed in univariate and multivariate analyses between blood fibrinogen concentration and pressure indices expressing severity of lower limb macroangiopathy (ankle/thigh, ankle/arm). In summary, increased levels of hematologic parameters contributing to the increase of whole blood viscosity were observed in Type 2 diabetics with arteriosclerosis obliterans. The observed relationship between fibrinogen concentration and severity of arteriosclerosis obliterans could be an evidence in favor for fibrinogen lowering therapy in this pathologic condition.
