Pathologic gait.

Abstract

The diagnosis of gait deviations and their causes requires three levels of knowledge. First is an awareness of the functional limitations caused by each type of abnormality (contracture, muscle weakness, pain, and spasticity). Second is familiarity with normal function. Third is having a trained eye or gait analysis system that is capable of identifying the gait deviations. Once diagnosis is complete, a fourth requirement is the ability to use the findings to set up an appropriate treatment regimen.

Serial MRI in Fukuyama type congenital muscular dystrophy.

Serial MRI of the brain of a female infant with Fukuyama type congenital muscular dystrophy (FCMD) is presented. Initial MRI revealed diffuse abnormal signal in the cerebral white matter extending peripherally. On follow-up studies, the abnormal signals disappeared or decreased from the posterior to anterior, and from central to peripheral. These changes in signal intensity correlate well with the process of myelination as demonstrated in histochemical studies. It appears that the abnormal signals in FCMD are caused by delayed myelination. When abnormal signal intensity is seen in the cerebral white matter of a developmentally delayed infant, serial MRI may be used to follow the course of the illness.

Mathematical modelling of the effect of sole elasticity distribution on pronation.

A coronal plane model of a distributed elastic sole has been proposed and analyzed with respect to the effects of different medial-lateral elasticity distribution on pronation under quasi-static conditions. The distributed model consists of an array of linear vertical line springs. Under minimum energy assumption, the behavior of the top surface of the interface under resultant force and moment loading was shown to be equivalent to that of a rigid-body mechanism under the same loading. The model was then combined with a rigid-link model of the lower limb. Expressions that describe the relationship of the interface aggregate parameters with pronation and the center of pressure were obtained. These expressions were confirmed by an experiment in which the elastic distribution in the interface was systematically varied and the pronation angle and the center of pressure measured. The model has the potential of being a useful analytical tool in the design of elastic soles in running shoes.

Patterns of relations between lower limb angle excursions during normal gait.

This paper deals with the analysis of data from a kinematic study of limb angle during normal gait. The first set of data deals with the variation of femur angle to the vertical, and of knee and ankle flexion-extension. The data are recorded on the System of Gestual Automatic 3D Analysis with a 50 Hz sampling rate. The second data set concerns morphological parameters. The data are obtained from 35 subjects. The time angle data are raw and are not to be related to a base position. The first stage of the angle analysis shows that a walking cycle can be characterized by 25 values. A factor analysis is then applied to these values for each angle in order to underscore the more informative instants. From this study, eight measurements are preserved. In the third stage, the same method is applied to both angles. It illustrates four ways to differentiate healthy subjects' walk. It also shows that the angles of femur and knee are strongly correlated and that there is a priori no clear relationship between the ankle and the two previous angles. The last stage is analysis of the morphological and angle parameters together. It brings to the fore the influence of sex and height for two groups of subjects.
