Advances in aesthetic surgery.

Abstract

The evolution of aesthetic surgery began centuries before the birth of Christ. Since Sushutra's first attempts, surgeons still try to achieve or reconstruct the perfection of individual beauty. Surgeons who are engaged in this field of plastic surgery are called cosmetic or aesthetic surgeons. Aesthetic surgery is not just a specialization concerned largely with the removal of the outward signs of aging, nor is it limited to plastic surgery for improvement by restoration of damaged areas of skin and removal of blemishes. Aesthetic surgery crosses the dividing line between surgery for reconstruction and alteration of deviations (which do not in themselves constitute objective deformities) and is sometimes even performed without medical indication, but just for the gratification of individual vanity. This article summarizes the progress of aesthetic surgery during the last decade in some aspects of the field and tries to estimate its' lasting value.

Technical tricks to improve the cosmetic results of breast-conserving treatment.

The paper describes different manoeuvres and surgical details that may help the general surgeon to improve the aesthetic outcomes after breast-conserving treatment for cancer. Among them, the pre-operative planning, the position of the patient in the operative room, the mobilisation of the glandular tissue when the tumour has been removed. All these manoeuvres contribute to achieving better symmetry with the healthy breast and to improve the final aesthetic result, minimising the surgical conflict between large resections and defects for optimal control of local disease and the consequent breast deformities.

Reconstruction of the nipple-areola complex: an update.

Along with continuing progress in reconstructive surgery of the breast numerous techniques of nipple-areola reconstruction have been developed. With time and experience some methods have been discredited to historical significance only while others have evolved to widely accepted concepts used by surgeons all over the world, which in turn contributed new ideas and modifications. In addition to those favourite techniques others are reserved as second-line alternatives in specific situations. The principle criterion for a pleasing nipple-areola complex is symmetry regarding several parameters: colour, texture, size, and projection. The purpose of this manuscript is to review and discuss the concepts and techniques of nipple-areola reconstruction that have evolved over the past decades. Furthermore, those principles and techniques are pointed out that fulfil best the criteria of an ideal nipple-areola complex with emphasis on different techniques of breast reconstruction and individual conditions of the patient.
