The hanging columella.

Abstract

A hanging columella significantly detracts from otherwise pleasing nasal esthetics. Preoperative assessment of the nasal base is crucial to determine the etiology of a pre-existing hanging columella, and to assess the potential for creating such a deformity with rhinoplasty. Prevention or correction of a hanging columella requires surgical techniques that take into consideration the anatomic tip dynamic factors involved. We discuss the anatomy of the nasal base, define the hanging columella, and distinguish this entity from others with which it can be confused. We propose a methodical approach to the evaluation of the hanging columella, and recommend corrective techniques which we have found successful in achieving good cosmetic results.

Technical tricks to improve the cosmetic results of breast-conserving treatment.

The paper describes different manoeuvres and surgical details that may help the general surgeon to improve the aesthetic outcomes after breast-conserving treatment for cancer. Among them, the pre-operative planning, the position of the patient in the operative room, the mobilisation of the glandular tissue when the tumour has been removed. All these manoeuvres contribute to achieving better symmetry with the healthy breast and to improve the final aesthetic result, minimising the surgical conflict between large resections and defects for optimal control of local disease and the consequent breast deformities.

Reconstruction of the nipple-areola complex: an update.

Along with continuing progress in reconstructive surgery of the breast numerous techniques of nipple-areola reconstruction have been developed. With time and experience some methods have been discredited to historical significance only while others have evolved to widely accepted concepts used by surgeons all over the world, which in turn contributed new ideas and modifications. In addition to those favourite techniques others are reserved as second-line alternatives in specific situations. The principle criterion for a pleasing nipple-areola complex is symmetry regarding several parameters: colour, texture, size, and projection. The purpose of this manuscript is to review and discuss the concepts and techniques of nipple-areola reconstruction that have evolved over the past decades. Furthermore, those principles and techniques are pointed out that fulfil best the criteria of an ideal nipple-areola complex with emphasis on different techniques of breast reconstruction and individual conditions of the patient.
