Silent periods in dysfunction patients: a preliminary study using a novel approach.

Abstract

Strong peri-oral stimulation is known to elicit a transient inhibition in the activity of the jaw-closing muscles. Several reports have claimed that this 'silent period' (SP) is prolonged in patients with temporomandibular joint dysfunction syndromes. In the present preliminary study, the biphasic SP evoked by electrical lip stimulation was tested in single motor units in the masseter muscles of dysfunction patients. In comparison with normal subjects, the pattern of the SP was different in the patients in that the shorter-latency phase predominated over the later phase, whereas the converse is true for normals.

[Training programs in stroke patients: literature review].

INTRODUCTION: An increased number of physical retraining programs are available for the rehabilitation of stroke patient; generally positive results are observed, but no consensus in methodology exists (program characteristics, intensity, duration, frequency, etc.). OBJECTIVE: To review the literature about training program characteristics for stroke patients and describe their efficacy, limits, and results. METHODS: A search of the Medline database revealed 97 references, including 53 clinical studies examining protocols of physical training and 15 meta-analyses and literature reviews of physical training. RESULTS: Three physical training protocols were found (walking, neuromuscular and 'force' training). Each training had specific effects: cardiovascular, functional, and muscular. DISCUSSION-CONCLUSION: To obtain positive cardiovascular effects and/or walking improvement, classical rehabilitation and physical training programs need to be associated for rehabilitation of the stroke patient.

Finding neuroelectric activity under magnetic-field oscillations (NAMO) with magnetic resonance imaging in vivo.

Neuroimaging techniques are among the most important tools for investigating the function of the human nervous system and for improving the clinical diagnosis of neurological disorders. However, most commonly used techniques are limited by their invasiveness or their inability to accurately localize neural activity in space or time. Previous attempts at using MRI to directly image neuroelectric activity in vivo through the detection of magnetic field changes induced by neuronal currents have been challenging because of the extremely small signal changes and confounding factors such as hemodynamic modulations. Here we describe an MRI technique that uses oscillating magnetic field gradients to significantly amplify and detect the Lorentz effect induced by neuroelectric activity, and we demonstrate its effectiveness in imaging sensory nerve activation in vivo in the human median nerve during electrical stimulation of the wrist. This direct, real-time, and noninvasive neuroimaging technique may potentially find broad applications in neurosciences.

Acid-induced release of platelet-activating factor by human esophageal mucosa induces inflammatory mediators in circular smooth muscle.

In a human in vitro model of esophagitis, we investigated the genesis of esophagitis-associated dysmotility by examining HCl-induced production of inflammatory mediators in the mucosa and investigating their effect on esophageal circular muscle. Muscularis propria was removed from organ donors' esophagi, leaving the mucosal tube intact. The tube was tied at both ends, forming a sac, and filled with HCl at pH 4. After 3 h of incubation, the supernatant surrounding the sac was analyzed or applied to circular muscle strips. HCl alone did not affect circular muscle contraction in response to electrical field stimulation (EFS), but supernatant of HCl-treated mucosa abolished contraction. The inhibition was reversed by the platelet-activating factor (PAF) antagonist CV3988 [(+/-)-3-(N-octadecylcarbamoyl)-2-methoxy) propyl-(2-thiazolioethyl) phosphate], whereas the PAF analog 2-O-methyl platelet-activating factor C-16 (PAF-16) inhibited EFS-induced contraction and acetylcholine (ACh) release in circular muscle strips. The hydrogen peroxide scavenger catalase reversed the inhibition in contraction, to the same extent as CV3988. We therefore measured PAF and hydrogen peroxide (H(2)O(2)) in mucosa, mucosa supernatant, and circular muscle. HCl increased PAF and interleukin (IL)-1beta (but not IL-6, prostaglandin E(2), or H(2)O(2)) in mucosa, and only PAF was released into the supernatant, presumably to affect circular muscle. In circular muscle, exogenous PAF induced sequential formation of IL-6, H(2)O(2), IL-1beta, and PAF. Release of PAF by the mucosa inhibits ACh release from circular muscle layer neurons and initiates sequential formation of inflammatory mediators in muscle, resulting in production of PAF by the muscle itself, possibly initiating in a self-sustaining cycle.
