Abstract

The result of the new plan, hopes Smeltzer, will be a dramatic reduction in the nursing shortage, at least at UCH. "We find this plan appeals to nurses on several levels," said Smeltzer. "First, it provides financial rewards, and those rewards affirm the Hospitals' commitment to treating nurses as highly valued professionals. Second, it will create a better working environment for nurses, with more full-time nurses working together as established teams. In the long-run, this nursing plan should lead to improved patient care. By reducing the nursing vacancy rate, encouraging nurses to work full time, and reducing the Hospitals' dependence on temporary nurses hired through registries, we should have better-trained, more experienced nurses caring for patients."

Forty four years of dermatophytes in a Chicago clinic (1944-1988).

Data are presented on 39,270 cultures taken over a 44 year span (1944-1988) at the University of Chicago's Dermatology Clinic. In the mid 1940's Microsporum audouinii accounted for 60-80% of isolates. It gradually decreased over the next two decades and disappeared altogether in the 1970's. Trichophyton rubrum, rare in the 1940's accounted for over 60% of isolates in the mid-1960's only to be overtaken by T. tonsurans. This species, not isolated till the mid 1950's, became and remains the dominant dermatophyte at the present time. Both T. mentagrophytes and Epidermophyton floccosum increased in the 1970's and decreased later. Unusual circumstances resulted in clusters of T. verrucosum, T. terrestre, and T. schoenleinii isolates. Infections were associated with rural dairy workers, zoo handlers and immigrant families respectively. M. canis and M. gypseum were steady at a low rate throughout the entire period. Rare isolates included M. cookei, M. persicolor, M. racemosum, T. simii, T. soudanense, T. violaceum, and the soil keratinophile, Aphanoascus fulvescens.

A comparison of clinical virulence of nosocomially acquired methicillin-resistant and methicillin-sensitive Staphylococcus aureus infections in a university hospital.

OBJECTIVES: To compare the clinical virulence of nosocomially acquired methicillin-resistant Staphylococcus aureus (MRSA) and methicillin-sensitive S aureus (MSSA) infections in 1989. DESIGN: A retrospective comparison of host factors, in-hospital exposures, sites of infections, and outcomes of patients with nosocomial MRSA and MSSA infections. SETTING: University of Illinois Hospital, Chicago, Illinois. PARTICIPANTS: Forty-four adult patients with nosocomial S aureus infections. RESULTS: The 22 MRSA-infected and 22 MSSA-infected persons were similar regarding mean age, gender, underlying diseases, and exposure to surgery. Before developing infection, MRSA-infected persons were more likely to have received antibiotics (73% compared with 27%, odds ratio = 7.1, 95% confidence interval [CI95] = 2.0-25.8 p = .003) and to have stayed in the hospital > 2 weeks (64% compared with 18%, odds ratio = 7.9, CI95 = 2.0-31.6, p = .002). Bacteremia was the most common presentation in the MRSA and MSSA groups (55% and 59%, respectively). Infectious complications and death were infrequent in both groups. CONCLUSIONS: MRSA and MSSA strains infect patients with similar demographic features and underlying diseases, but MRSA infections are significantly more common among patients with previous antibiotic therapy and a prolonged preinfection hospital stay. Clinical presentations and outcomes did not differ significantly between the 2 groups. Thus, similar to studies in the early 1980s, our findings do not suggest greater intrinsic virulence of MRSA.
