Fixed prosthodontics centric relation registration technique using resin copings.

Abstract

The registration of an accurate centric relation is one of the most critical steps in a prosthodontic rehabilitation, yet it sometimes is an uncertain procedure. A technique using resin copings, each supporting an occlusal contact, is presented. By careful, selective addition of resin, the centric relation recording can be controlled. The technique requires a pattern of abutments to provide stability, and is, therefore, primarily applicable to fixed prosthodontics. Nonetheless, it is versatile enough to accommodate a variety of clinical situations.

Image analysis and occlusion.

This study was designed to investigate the measurement of the occlusal contact area (surface) using commercially available equipment and computer programs for image processing and analysis. A stable occlusal record was made using an addition curing silicone rubber putty and was placed on a light source onto which a video camera was focused. This image was digitized and standard thickness records were used to calibrate the appropriate threshold for measurement of contact and near contact surfaces. Differences between individuals and within individuals with and without their partial dentures are large. This method has the potential to be a powerful tool in the study of occlusion.

Anatomy of the temporomandibular joint and related structures in the frontal plane.

Conclusions about the anatomy and physiology of the temporomandibular joint have too often been based on sagittal representations. The aim of the article is to describe frontal serial cuts of an acrylic embedded articulation of a subject with teeth in intercuspal position. The frontal plane is best to visualise the difference between lateral and medial aspects of structures such as the disk-condyle-complex, the articular capsule, and related muscles.

Control of vertical position of the maxilla in orthognathic surgery: clinical application of the sandwich splint.

The Le Fort I osteotomy can be used to change the position of the maxilla in all three dimensions. A presurgically fabricated occlusal splint is used to reproduce the planned sagittal and transverse position of the maxilla. Various methods for determining the proper vertical position have been described. The major difficulty in vertical control when these techniques are used is the sagittal change of the lower reference point as the maxilla is advanced. The relative error in the vertical plane increases with larger sagittal movements. The sandwich splint technique, with two stable intraoral reference points, can be used to ensure proper vertical positioning of the maxilla. This method enables exact measurement of the vertical dimension during cast surgery and in the operating room. Vertical measurements are based on the relationship between the mandible and skull base. This distance remains constant during splint fabrication and when the splint is used at surgery. Postoperatively, the mandible can be rotated into the new centric occlusion.
