Bone and soft tissue procurement.

Abstract

Most health professionals are aware of the criteria that need to be met to secure organ donation. Bone and soft tissue procurement is another issue, however. Even though allografts have been used since the 1880s, new techniques and an increase in the possible uses for allografts have recently increased the demand for bone and soft tissue. The criteria for choosing a bone donor is similar to organ donors with one major exception. A bone and soft tissue donor does not need the special circumstances of brain death.

Seven-year follow-up of vertebral excision and reconstruction for malignant hemangioendothelioma of bone.

STUDY DESIGN: This study reports the 7-year follow-up of an L5 vertebral hemangioendothelioma treated with tumor excision and allograft reconstruction stabilized with transpedicular fixation. OBJECTIVES: A review of vertebral hemangioendotheliomas is provided to outline the rationale for surgical excision in such cases. SUMMARY OF BACKGROUND DATA: Radiation therapy has been the mainstay of therapy in surgically inaccessible lesions of the spine. The long-term follow-up of radical spinal tumor excision and reconstruction in previously surgically inaccessible areas has not been reported for vertebral hemangioendothelioma. METHODS: After embolizations, anterior L5 corpectomy and allograft femoral reconstruction was performed. Second-stage (same day) posterior element excision was followed by VSP stabilization of L4-S1 with artificial pedicles anchored into the femoral allograft at L5. Radiation therapy followed. RESULTS: Satisfactory long-term (7-year) segmental reconstruction using allograft and VSP was realized, even in the face of postoperative radiation therapy. CONCLUSION: Contemporary spinal reconstructive techniques have been developed that continue to limit the number of surgically inaccessible tumors in the spine. The present report shows these techniques to be durable and apparently effective in vertebral hemangioendothelioma.

Anterior cervical foraminotomy and fusion. Surgical technique and results.

STUDY DESIGN: This study retrospectively reviewed the clinical and radiographic results of a series of patients who underwent osteophyte resection during anterior discectomy and fusion. OBJECTIVE: To determine whether resection of the posterior uncinate process at the time of anterior cervical discectomy is safe and effective. SUMMARY OF BACKGROUND DATA: Previous studies have described techniques of osteophyte resection, but safety and outcome have not been supported by a clinical series of patients. METHODS: Forty-three consecutive patients with radiculopathy due to spondylosis or lateral herniated nucleus pulposus had 68 foraminotomies done at the time of anterior cervical disc foraminotomy for radiculopathy. All patients were followed-up until radiographic union. All patients were examined, and Odom's criteria were used to assess postoperative pain and function in the neck, arm, and iliac donor site. RESULTS: The results were consistent with previous reports of anterior cervical disc foraminotomy for radiculopathy. Excellent and good results occurred in 77% and 14% of the patients, respectively. Three patients had no relief and no patient had worsening of radiculopathy. The fusion rate was 93%. Thirty-one patients (72%) reported being very satisfied. Three patients required a re-operation--one for bone graft dislodgment and two for pseudoarthrosis. No vascular injuries occurred. CONCLUSIONS: Resection of the posterior portion of the uncinate process to remove osteophytes that narrow the neural foraman or to allow direct visualization of laterally herniated disc material can be done safely. When combined with an autogenous interbody bone graft, good results can be expected.

[5-year experience with a central bone bank].

Since December 1988 the Leiden Bone Bank Foundation, in cooperation with the BIS Foundation, makes allogeneic bone and related soft tissue available for transplantation. Bone banking has become a scientifically high level medical activity in which international standards are established in order to provide safe and effective allografts. Careful donor selection and extensive laboratory testing are the cornerstones for the prevention of disease transmission. In the first 5 years of the existence of the Leiden Bone Bank 450 deep frozen massive bone allografts, of which 221 osteoarticular and intercalary allografts, were shipped to different clinics inside and outside the Netherlands. Furthermore, 322 deep frozen soft tissues, 1877 units of freeze dried bone grafts and 5629 units of demineralized grafts were distributed. The number of transplants with allogeneic bone and related soft tissue from the Leiden Bone Bank increased each year by at least 30% Bone and related soft tissue allografts were used in orthopaedic surgery, neurosurgery and maxillofacial surgery. There are no known cases of transmission of infectious diseases by grafts distributed by the Leiden Bone Bank.
