Dermatomyositis.

Abstract

Pregnancy in DM/PM is infrequent probably because of the late age of onset of the disease. When pregnancy occurs in patients with long-standing remissions of childhood onset DM/PM the risk of exacerbation is 40 per cent and the prognosis for the fetus is reasonably good. If pregnancy starts in patients already diagnosed and under steroid treatment of DM/PM, the frequency of exacerbations is low (16 per cent) and the outcome of pregnancy is not as good, with only 50 per cent at-term babies and 37.5 per cent fetal loss. When DM/PM has its onset during pregnancy the fetal mortality reaches 62 per cent. No maternal deaths have been reported. The response to steroid therapy for active DM/PM during pregnancy seems to be good with two patients recovering their muscle strength very rapidly after delivery. There are no placental abnormalities that could explain the increased fetal loss. The newborns have no stigmata of the maternal disease. Pregnancy in DM/PM should be considered a high-risk pregnancy and managed accordingly; adequate prednisone therapy for the maternal disease, frequent monitoring of the growth and development of the fetus, careful search for signs of fetal distress and interruption of pregnancy as needed with the care of a neonatologist at the time of delivery. The mechanisms of the reciprocal interaction between DM/PM and pregnancy are unknown.

[Informative value of varying serum alpha fetoprotein levels in pregnant women based on a screening program in a Hungarian institute. What next?].

The authors evaluated the maternal serum alfa fetoprotein screening data of 3 years period. Out the neural tube defects they observed increased values at 20% of another congenital malformations. At the pregnant having abnormal serum alfa fetoprotein significantly accumulated the late-term abortion, stillbirth and preterm delivery. At the mothers of stillborns and abortions frequently noticed the different values. They direct attention to the fetal-living-danger importance, control functions of the abnormal level and after prospective data collection it may have new aspecific alarm role too. Accordingly the maternal serum alfa fetoprotein may consider like fetal blood sediment examination.

Characterization of the spectrum of alternative splicing of alpha 1 (XIII) collagen transcripts in HT-1080 cells and calvarial tissue resulted in identification of two previously unidentified alternatively spliced sequences, one previously unidentified exon, and nine new mRNA variants.

Amplification of a COL1-encoding region of alpha 1 (XIII) collagen transcripts of HT-1080 cell RNA suggested that exon 3 of the alpha 1 (XIII) collagen gene, which was previously deduced to be of 35 base pairs (bp) may consist of a constitutive 8-bp exon and an alternatively spliced 27-bp exon, termed here exons 3A and 3B, respectively. Furthermore, a previously unidentified alternatively spliced Gly-Xaa-Yaa-encoding exon designated as 4B was found between the sequences encoded by exons 4, redesignated here as 4A and 5. Six of the 16 potential combinations of the four consecutive alternatively spliced exons 3B, 4A, 4B, and 5 were found to exist in mRNAs, and as a result, the length of the COL1 domain may vary between 57 and 104 amino acid residues. Most of the NC2 domain is encoded by the alternatively spliced exons 12 and 13. Where previous analysis of cDNAs indicated that mRNA variants exist that contain either exon 12 or 13 sequences, amplification studies indicated here that there are also variants that lack both exons 12 and 13 but none that contain both exons simultaneously. Thus, the predicted length of this domain is either 12, 31, or 34 residues. Analyses covering both the COL1 and NC2 domains demonstrate that at least 12 mRNA species exist through the alternations of exons 3B-5, 12, and 13.
