Abstract

Human bite injuries, as innocuous as they may initially appear, may result in severe impairment. With a better understanding of the anatomy, microbiology, mechanism of injury, and clinical presentation, early diagnosis and the institution of appropriate therapy may minimize the morbidity commonly associated with human bite infections of the hand.

[Travel and skin diseases].

The problem "travelling and dermatological diseases" is presented as a temporary change of place with associated changes in ecological conditions. Latent dermatoses may be provoked--but full-blown dermatoses may also improve with no specific treatment (climatic therapy of neurodermatitis). Physiological changes at the surface of the skin brought about by, for example, temperature or the effects of solar radiation, may allow fungal, bacterial or viral infections to develop. Direct contact with the living environment on land or in the water, in particular in the tropics, can lead to the development of diseases. Some dermatoses have a lengthy latency and develop only later at home. Recommendations for general and specific prophylaxis and treatment are made.

Upper extremity wound management: intermediate infections.

A careful history and physical examination, including information regarding position of the hand at the time of injury, presence of other disease, handedness, age, and occupation, are all essential to evaluate hand infection. If surgery is anticipated, antibiotics should generally be withheld until operative cultures are obtained. Infections secondary to human bite injuries are common and are usually caused by a blow of the fist to the mouth. These injuries can produce crippling sequelae, must be managed promptly and aggressively, and often require surgery. Flexor tenosynovitis, often caused by a penetrating injury, may be treated nonoperatively if the patient presents within 24 to 48 hours of onset. Older injuries always require incision and drainage of the flexor tendon sheath. Most authors agree that surgical management of dog bite wounds to the hand is essential. Cat and arthropod bites may also require surgery.

Fish stings and other marine envenomations.

West Virginia, it would seem, is an unlikely place for physicians to encounter patients with poisonous marine envenomations. To the contrary, West Virginias who vacation at the beach may be envenomated and require further evaluation and treatment when they return home. Likewise, certain aquarium pets or even freshwater fish may envenomate those who have contact with them. Such underwater sea creatures can cause local and systemic toxic or allergic reactions which potentially can be serious. This article describes these possible toxic encounters as well as first aid and medical management.
