Abstract

The results of 50 shoulder arthroscopies are presented. A great variety of different lesions were seen. In 27 of the 50 cases the diagnosis was changed or refined. In particular, unexpected rotator cuff tears and radiolucent loose bodies were found. The presence of Bankart and Hill-Sachs lesions confirmed the diagnosis of recurrent dislocation in clinically subluxing patients with no radiological evidence of dislocation. Shoulder arthroscopy promises to increase diagnostic precision in patients with shoulder pain and may have therapeutic potential with minimal morbidity.

Synovial haemangioma of the knee joint.

Synovial haemangioma of joints are rare, but usually affect the knee joints of children and adolescents. Recurrent swelling, usually due to haemarthrosis, and intermittent pain may be present for a long time before the diagnosis is made. In this case report the common features of the condition are described, with emphasis on the occurrence of atraumatic haemarthrosis in children. The value of arthroscopy in diagnosis is discussed.

Unnecessary arthroscopies.

A review of the results of arthroscopies on patients taken from an 'open access' service waiting list has been carried out. One-third of all arthroscopies proved to be negative. Accuracy of clinical diagnosis was not related to the clinical grade of the examining doctor. The excess annual cost to the Northern Ireland Orthopaedic Service of unnecessary arthroscopies may have been of the order of 89,000 pounds.

Rehabilitation of the temporomandibular joint after arthroscopic surgery.

Arthroscopy of the temporomandibular joint (TMJ) is a new, relatively noninvasive surgical procedure for treating patients with TMJ dysfunctions beyond management by nonsurgical means. Ninety-two patients who underwent diagnostic and operative TMJ arthroscopy completed a staged physical therapy regimen, which resulted in functional mobility and mandibular use with little or no pain at 24-month follow-up. This report introduces TMJ arthroscopic diagnostic and surgical procedures and their respective rehabilitative protocols currently used to provide beneficial outcomes in postoperative treatment.
