The mechanism of gingival overgrowth associated with long-term use of nifedipine and of other drugs that affect calcium homoeostasis, such as cyclosporin and phenytoin, is unknown. With an ultrasensitive assay, we measured the pharmacokinetics of nifedipine in plasma and gingival crevicular fluid (GCF) of nine patients receiving this drug for angina and hypertension. In seven patients, the maximum nifedipine concentration was in the range 15-316 (mean 84 [SD 105]) times greater in GCF than in plasma. The two patients with low (undetectable) GCF nifedipine did not have overgrowth. We propose that gingival tissues sequester nifedipine and that the very high nifedipine concentrations predispose the tissues to overgrowth.
Congenital fusion of the gums is a rare abnormality. This is a report of a successfully treated newborn infant who had unilateral fusion of the gums. A brief review of the previously reported cases is also presented.
The toothlessness and, specifically, the oral gap which is present between the lower and upper gums in the oral cavity of the toothless baby, while only temporary anatomical deficiencies, are, nevertheless, very important ones. It is suggested herein that this phenomenon of toothlessness and temporary physical abnormality has the ability to cause sudden infant death syndrome (SIDS) in a significant number of babies in their early months. What follows is a presentation of a new biological approach to the underlying causes of SIDS and, in the Conclusion section, concrete and practical suggestions are given for a completely new method to counteract and neutralize the threatening dangers of SIDS.
