Septum primum defect repair.

Abstract

Two technical maneuvers in septum primum defect repair have resulted in improved mitral valve function and security against heart block. One maneuver, critical to restoring mitral competence, is accurate reconstruction of the leading edge of the mitral valve. A second maneuver, suturing the septal defect patch at its inferior aspect to the mitral valve near the annulus instead of to the inferior septal defect margin or to the tricuspid annulus, eliminates the hazard of atrioventricular node or bundle injury.

Cardiac involvement in Kugelberg-Welander disease: a case report and review.

There are few reports of cardiac involvement in patients with Kugelberg-Welander disease. We report a case of a 51-year-old man with Kugelberg-Welander disease who presented with syncope. His electrocardiogram showed complete right bundle branch block and transient complete atrioventricular block without escape rhythm. He was successfully treated with emergency temporary pacing followed by permanent pacemaker implantation. In this report, we review the relevant literature and argue that patients with Kugelberg-Welander disease should be evaluated regularly for cardiac disease.

Transient complete atrioventricular block associated with herb intake.

We report a case of transient complete atrioventricular block in a 38-year-old man, after intake of a mixture of herbs, intended to aid cigarette smoking cessation. Since all other causes of conduction disturbances were excluded, a side-effect of the herbal remedy was identified as the most likely diagnosis. Given that most patients are unaware of the potential risks of the intake of various herbs, we would urge that their usage be regulated.
