[Venous angioscopy].

Abstract

Vascular endoscopy, especially of the venous system, allows a tri-dimensional evaluation and represents, to date, the best possible per-operative control of the diameter of the vessel. Under these conditions, venous thrombectomy may be performed more accurately; residual adherent parietal thrombi which may represent a possible source of emboli, may be eliminated. Venous endoscopy may be repeated several times; it does not result in any X-Rays exposure for the patient and enables to diagnose finer morphological details. Therapeutic measures under direct vision could even become possible in the future.

[Laryngomalacia--endoscopic microsurgical treatment].

Laryngomalacia is caused by inspiratory collapse of the supraglottic structures in babies. This predominant cause of stridor in the first years of life has a graded occurrence from an insignificant sound phenomenon to insufficient alveolar ventilation with possible serious complications and development of dystrophy. The laryngomalacia must be sub-classified by direct laryngoscopy with a flexible endoscope under spontaneous respiration. Microsurgical techniques for performing supraglottoplasty have recently been introduced. The authors describe symptoms and results in four cases of laryngomalacia, in one of whom a supraglottoplasty was performed. The authors emphasise the importance of recognising laryngomalacia as a possible cause of delayed development in the first year of life.

Importance of fungus colonization in failure of silicone rubber percutaneous gastrostomy tubes (PEGs).

Silicone rubber PEG tubes or replacements were recovered from 111 patients and examined for blockage, dilatations, tears, breaks, or loss of elasticity. All irregularities were stained and examined for fungus using lactophenol cotton blue stain. The intraabdominal portion of the PEG failed from obstructions, loss of elasticity, or tears related to fungus colonies in 36% of cases. An additional 34% were colonized with fungi but did not fail. On frozen section, the fungus invaded the wall of the tubing. The extraabdominal PEG tubing failed from fungi in 12, and 10 additional tubes had colonizations. Nine tubes had distal clogging with crystalline material that is believed to arise from medication. Fungus tube failure occurred in 37% of the tubes in place 250 days and in 70% of tubes in place 450 days. Fungus is an important cause of PEG failure; recommendations are provided to maintain tube patency.
