Cardiac pacemaker emergencies.

Abstract

Each component of a pacemaker system is a potential weak link in the pacing chain. Whether or not pacing failure is caused by battery depletion or by electrode fracture, these malfunctions can be diagnosed with the help of intermittent and regular transtelephonic surveillance. Sufficiently large numbers of patients enrolled in such surveillance systems enable the early identification of faulty pacers and leads and alert the medical community against their use. Indeed, the pacemaker surveillance systems in operation at present form the cornerstone of defense against pacemaker malfunction emergencies.

Supported liquid membrane coupled on-line to potentiometric stripping analysis at a mercury-coated reticulated vitreous carbon electrode for trace metal determinations in urine.

A method for trace metal determinations in complex matrices is presented. The method combines supported liquid membrane (SLM) sample clean-up and enrichment with potentiometric stripping analysis (PSA) in a flow system using reticulated vitreous carbon (RVC) as the electrode material. The membrane contained 40% m/m di-2-ethylhexylphosphoric acid dissolved in kerosene. Lead was used as a model substance in high-purity water and urine samples. The samples were enriched after a simple pH adjustment. The SLM enrichment time was 10 min when the last 5 min electrodeposition on the RVC electrode at -1.0 V (versus Ag/AgCl) was performed simultaneously. The influence of various experimental parameters such as deposition time, deposition potential and flow rate on the lead signal was investigated. With a 10 min SLM enrichment including a 5 min deposition time, the detection limit for lead was 0.3 microgram l-1. The relative standard deviation for lead concentrations in the range 4-20 micrograms l-1 was 0.05%. The overall SLM-PSA system was found to be stable for at least 100 urine analyses. The method was validated by running a reference urine sample. The result obtained (five replicates) was 9.7 micrograms l-1 (standard deviation 1.8 micrograms l-1) which is within the recommended range of 9.2-10.8 micrograms l-1.

On the realization of an analytic high-resolution EEG.

The analytic solution of the harmonic downward continuation of the scalp potential field in an N-shell heterogeneous, but isotropic, spherical volume conductor model has been derived. The objective of this paper was to investigate the realization of a so-called "high-resolution electroencephalogram (EEG)": by enhancing the poor spatial resolution of EEG recordings. To this end, the forward problem for a dipolar source arbitrarily located at the source point Q = Q(rs, phi s, theta s) has been determined in a compact matrix notation. It is possible to transfer the potential field given on the outer surface of a spherically shaped volume conductor to an arbitrary inner surface (e.g., to the cortical surface) under consideration of the electrical and geometrical properties of the model. For the application of the proposed method to real-world problems, the coefficients of the series expansion describing the cortical potential distribution are determined by minimizing the squared curvature of the scalp potential field integrated over the scalp surface. Simulation results for distributed sources show that the proposed method is superior to the surface Laplacian method for interelectrode distances below 2.5 cm.

[Electrodiagnosis of focal neuropathies].

Electrodiagnostic techniques used in an EMG-Laboratory and the diagnostic value in focal mononeuropathies are presented. The electrodiagnostic techniques include nerve conduction studies and needle electrode examination. Measurement of distal latency, amplitude, conduction velocity and the finding of focal conduction block may localize the focal character of the mononeuropathy. The needle electrode examination gives further information about distribution, extent, activity, or chronicity of the disease, especially in cases with axonal injuries. Some normal electrodiagnostic patterns are presented, accompanied by some illustrative pathological cases.
