[Amnioinfusion or not during labor? Better documentation and more trials can answer the question].

Intrapartal amnio-infusion has been described as a means of reducing the risk of fetal distress, as well as the Caesarean section rate among cases of oligohydramnios accompanied by CTG (cardiotocography) signs of cord compression. Among cases complicated by the presence of thick, meconium-stained amniotic fluid, dilution by amino-infusion has been shown to reduce the prevalence of meconium below the vocal cords, and of meconium aspiration syndrome in the newborn. A questionnaire concerning experience of the method, answered by 98% (61/62) of delivery wards in the country, showed Swedish obstetricians to be generally in favour of the procedure which they considered beneficial. However, as many of the wards had had very few cases, obstetricians answering the questionnaire felt that there was a need of guidelines and better prospective clinical studies.

Evidence for the presence of dopamine D1 receptor mRNA and binding sites in monkey amniotic epithelial cells.

In this study we examined the presence of dopamine D1 receptors in monkey amniotic epithelial cells (MAEC) using RT-PCR and radioligand binding experiments. We found that MAEC express D1 receptor mRNA that is having 99% homology with human dopamine D1 receptors. Saturation binding studies using [3H]SCH-23390 showed a high affinity D1 site with K(D) and Bmax values of 0.82 +/- 0.12 nM and 20.77 +/- 4.22 fmol/mg protein, respectively. Competition experiments showed that selective D1, but not D2, antagonists are potent displacers of [3H]SCH 23390 binding with a rank order of potency that is consistent with the pharmacology of the dopaminergic D1 site. These data provide, for the first time, compelling evidence that MAEC natively express D1 mRNA and binding sites and suggest that it may be a potential primate cell model to study D1 receptors and to explore new selective drugs active at these receptors.

Correction of hemorrhage-induced anemia with intra-amniotic iron in the ovine fetus.

OBJECTIVE: This study tested the hypothesis that intra-amniotic iron treatment would enhance fetal red blood cell production after an acute, severe fetal hemorrhage of 40% of estimated blood volume over 2 hours. STUDY DESIGN: Three groups of late-gestation ovine fetuses were studied for 10 days: (1) control fetuses (n = 8), (2) fetuses hemorrhaged on day 3 (n = 11), and (3) similarly hemorrhaged fetuses supplemented with a single bolus of 60 mg of iron injected intra-amniotically immediately after the hemorrhage (n = 7). Statistical analysis was by 3-factor analysis of variance. RESULTS: At 24 hours after hemorrhage, red blood cell mass increased 5% in the control group and was reduced equally in both hemorrhage groups by 32% below day 3 prehemorrhage values. At 7 days after hemorrhage, red blood cell mass increased 27.8% +/- 2.6% (SE) above day 3 baseline values in the control fetuses. In the nonsupplemented hemorrhaged fetuses, red blood cell mass was not different from prehemorrhage values after 7 days (+3.7% +/- 4.1%), whereas red blood cell mass increased by 29.9% +/- 6.1% above prehemorrhage values in the iron-supplemented hemorrhage group (P <.001). CONCLUSION: Intra-amniotic iron supplementation resulted in full restoration of red blood cell mass within 7 days after a large loss of blood in fetal sheep, whereas restoration failed without iron supplementation. Intra-amniotic iron treatment may be of therapeutic value in restoring red blood cell mass in human fetuses with certain types of anemia such as that resulting from fetal or fetomaternal hemorrhage.
