Abstract

G6PD, GSH-Px and 6PGD activity was found to be elevated in beta-thalassemia heterozygous erythrocytes. Estimates of the activities of the three enzymes in red cell fractions of differing mean age separated by centrifugation through a density gradient of Ficoll-Triosil layers, showed that the rate of in vivo decline was normal for G6PD and GSH-Px, and decreased for 6PGD. The increased enzyme activity results from increased rate of synthesis for G6PD and GSH-Px, and from higher in vivo stability for 6PGD.

Routine transfusion of Rh(D)-positive RBCs to Rh(D)-negative patients designated as do not resuscitate conserves Rh(D)-negative red blood cell inventory.

BACKGROUND: A minority of blood donors are Rh(D)-negative, and Rh(D)-negative red blood cell (RBC) products are often overutilized. As such, Rh(D)-negative RBCs may be difficult to maintain in blood bank inventory. STUDY DESIGN AND METHODS: We changed our blood bank laboratory policy to approve non-alloimmunized Rh(D)-negative patients to receive Rh(D)-positive RBCs for routine transfusion under defined criteria. Those criteria included Rh(D)-negative males (all ages) and females (aged >50 years) who were designated as do not resuscitate (DNR), either with or without intubation, in the electronic medical record. RESULTS: From August 15, 2024 through August 15, 2025, a total of 204 Rh(D)-negative patients met the above criteria and were approved to receive routine Rh(D)-positive RBC transfusions. Within that group, 23 patients received Rh(D)-positive RBCs. The remaining patients either did not require transfusion or were issued Rh(D)-negative RBC units. Since implementing this practice, a total of 68 Rh(D)-negative units were conserved during this time frame. Notably, 28 of the 68 units (41%) were type O, Rh(D)-negative. DISCUSSION: Rh(D)-positive RBCs can be routinely given to non-alloimmunized Rh(D)-negative patients who are not at risk for developing hemolytic disease of the fetus and newborn (HDFN). By creating clear guidelines for the routine administration of Rh(D)-positive RBCs to patients who are not at risk for HDFN, the inventory of Rh(D)-negative RBC units can be directed to those patients who would most benefit from this limited resource.

[Red cell filterability, smoking and cardiovascular risk factors (author's transl)].

Red cell deformability, which allows cells of 7 mu diameter to flow through capillaries not larger than 3 mu, can be approached by the measure of blood filterability on nuclepore 5 mu filters. Filterability is reduced in arterial diseases. We have, in 72 patients, correlated red cell filterability, with the number of cardiovascular risk factors present high blood pressure, overweight, diabetes, hyperuricemia, hyperlipemia smoking). There is a statistical difference between groups with risk factors present as a whole and with O risk factor (p less than 0.01). The difference is highly significant between O and 4 risk factors (p less than 0.0005). Filterability decrease is also directly correlated with the number of cigarettes smoked per day (less than 0.05) and decrease is enhanced by smoking two cigarettes.

Interference of the measurement of lactate dehydrogenase (LDH) activity in human serum and plasma by LDH from blood cells.

This study was performed to find a reliable method to measure lactate dehydrogenase (LDH) activity in blood samples. Human plasma contains thrombocytes whose number depends on the speed of centrifugation applied to the freshly-drawn blood sample. They do not disturb the measurement of LDH in plasma, provided the plasma sample has not been subjected to procedures that may destroy the integrity of the thrombocytes, e.g. standing overnight at 4 degrees C, freezing, or treatment with ultrasonic waves. Centrifugation of plasma at a minimum of 1174 x g eliminates the presence of thrombocytes. The serum of healthy human subjects invariably contains higher LDH activity than the plasma: the difference is 30 +/- 18 U/l (mean +/- S.D.). LDH-isoenzyme analysis shows that a large part of the LDH activity difference is caused by lysis of erythrocytes in the clot. Liberation of 30 U LDH per litre is not associated with visible hemolysis.
