A prospective study in an area with endemic goitre showed 18 patients (15.1%) out of 119 investigated with iodine containing contrast media to be hyperthyroid after 28 days. Oral cholecystography led to the highest (27.9%) and infusion urography to the lowest (5.3%) rate of hyperthyroidism. Determination of the serum concentration of thyroxine, triiodothyronine, basal and stimulated thyrotropine is not a suitable method to detect the risk for hormonally euthyroid patients before a contrast media investigation.
Seven patients who had typical and persistent symptoms of gallbladder disease but repeatedly normal oral cholecystograms were seen in an 18 month period. Ultrasound examination of the gallbladder was diagnostic of cholelithiasis in all seven patients. Six patients were found to have gallstones at operation, and the seventh had histologically proved chronic cholecystitis. Although oral cholecystrography is a highly accurate examination, false-negative results do occur. Gray scale ultrasonography is a safe and also highly accurate means of providing objective evidence of cholelithiasis in patients clinically thought to have gallstones despite normal oral cholecystograms.
No differences could be detected subjectively in the quality of cholangiograms after the use of varying amounts of contrast medium (20 and 30 ml iodoxaminic acid). Pharmacolinetic data which would indicate the use of either quantity are not available. Densitometric measurements carried out by us showed no significant statistical difference between the results of using 20 or 30 ml iodoxaminic acid.
