[Efficient procedure for diagnosing abdominal abscesses using imaging technics].

Abstract

In 56 patients with abdominal abscess typical radiological abscess sings were analized to establish a rational and stepwise use of imaging procedures. Plain radiographs of the abdomen should be carried out at first because of their large diagnostic possibilities and standardized practicability. In addition with ultrasonography the diagnosis is possible in almost 90% of the cases. Contrast medium examination are rarely of diagnostic value. Computed tomography should be used in the remaining unclear cases and for planning of further surgical treatment.

[Extrapulmonary tuberculosis. An important differential diagnosis in immigrants with suspected malignancy].

The incidence of tuberculous disease is increasing all over the world, mostly in the poor, developing countries, but also in some industrialized countries. In Norway, extrapulmonary tuberculosis is a rare phenomenon. It is found mostly among older Norwegians and in younger immigrants from the third world. Since the disease is rare, it may be overlooked or confused with malignant disease. We describe two patients with unusual forms of extrapulmonary tuberculosis, both mimicking neoplastic disease. The first patient was a 27-year-old woman from South-East Asia, who was operated on for suspected intraductal comedo-type carcinoma of the breast, but histological examination showed tuberculous mastitis. The second patient was a 26-year-old man from East Africa with a medical history indicating intra-abdominal lymphoma. The final diagnosis, however, was mesenteric tuberculous lymphadenitis. Both patients were treated successfully with isoniazid, rifampicin and pyrazinamide.

Microsurgical excision of an abdominal mass in a gourami.

A female, sunset, thick-lipped gourami (Colisa labiosa) that weighed 8 g and was 5.4 cm from the snout to the end of the vertebral column was examined because of a 5-mm-diameter, midventrally located mass that had developed suddenly 1 month earlier. Cytologic examination of a sample obtained by use of fine-needle aspiration, survey radiography, positive-contrast radiography of the gastrointestinal tract, and Doppler ultrasonography were performed to evaluate the mass. These procedures were not able to provide a definitive diagnosis, but did facilitate surgical planning. The mass was excised, and the abdominal musculature was repaired, using microsurgical techniques. Redevelopment of the mass was not detected during the 5-month period after surgery. Histologic evaluation of the mass revealed an organized hematoma of undetermined cause. Successful management of this gourami illustrated that size should not be a deterrent to diagnostic evaluation and surgical intervention in diminutive fish.
