Abstract

True Vleminckx's solution incorporated in a natural clay-mask vehicle apparently retains its therapeutic qualities while relinquishing all unaesthetic properties. A double-blind, randomized clinical trial comparing the Vleminckx mask to its vehicle was conducted. While the mask vehicle may not be considered a placebo, only the active drug produced a statistically significant reduction in comedone count at the conclusion of the study. The Vleminckx mask achieved a substantially greater reduction in papule count than the vehicle (p less than 0.05). Male and female patients responded positively to the concept of every-other-evening applications of only 20 minutes, as well as to the mask's rapidly astringent and cooling skin effects.

Adrenocortical oncocytoma in a child.

Adrenocortical oncocytoma is a rare epithelial tumor only described in adults. We report the case of a 12-year-old female who presented a left adrenal mass with abdominal pain, fatigue, acne vulgaris, and elevation of the androstenedione and total testosterone. She had an adrenalectomy. A diagnosis of adrenocortical oncocytoma was made after detailed histological, immunohistochemical, and ultrastructural studies.

Two randomized studies demonstrate the efficacy and safety of dapsone gel, 5% for the treatment of acne vulgaris.

BACKGROUND: A new aqueous gel formulation of dapsone has been developed that allows clinically-effective doses of dapsone to be administered topically with minimal systemic absorption. OBJECTIVES: The goal of these studies was to evaluate the efficacy and safety of dapsone gel, 5% in the treatment of acne. METHODS: Patients 12 years of age and older with acne vulgaris (N = 3010) participated in two identically-designed 12-week, randomized, double-blind studies of twice-daily monotherapy with dapsone gel, 5%, versus a vehicle gel. RESULTS: Dapsone gel-treated patients achieved superior results in terms of the investigator's global acne assessment (P < .001) and the mean percentage reduction in inflammatory, noninflammatory, and total lesion counts (all, P < .001) at week 12. Reductions in inflammatory lesion counts favoring dapsone gel over vehicle were apparent as early as 2 weeks and reached statistical significance by 4 weeks. No clinically significant changes in laboratory parameters, including hemoglobin, even among glucose-6-phosphate dehydrogenase-deficient patients, were observed. Adverse events were comparable between the treatment groups and rarely led to discontinuation. LIMITATIONS: Adjunctive topical treatments and their impact on acne were not studied in this trial. CONCLUSIONS: Dapsone gel, 5% appears to be an effective, safe, and well-tolerated treatment for acne vulgaris, with a rapid onset of action.

Acne and PDT: new techniques with lasers and light sources.

Acne vulgaris is one of the most common dermatologic disorders encountered in everyday practice. Treatment options for this often psychologically scarring disease are numerous and, for many individuals, provide relief from the disorder. However, factors such as antibiotic resistance and slow onset of action from many topical therapies have led researchers to seek out alternative therapies, especially for those suffering from moderate to severe inflammatory acne vulgaris. Lasers and light sources are finding increased usage in the treatment of inflammatory acne vulgaris. Light sources including blue lights and intense pulsed lights are becoming regular additions to routine medical management to enhance the therapeutic response. Finally, photodynamic therapy may change many of the acne vulgaris paradigms, as its place is being defined in the treatment of moderate to severe inflammatory acne vulgaris. More and more clinical trials are showing the effectiveness of 5-aminolevulinic acid and photodynamic therapy in the treatment of moderate to severe inflammatory acne vulgaris.
