Cognitive difficulties in psychoanalysis.

Abstract

The author rises a number of questions about cognitive difficulties in relation to psychoanalysis. He presents the case of an adult patient in whom previously unrecognized childhood cognitive difficulties were discovered during the course of analysis. Their relationship to the patient's problems in adulthood is discussed. Some suggestions and speculations about the questions raised are presented with the hope of stimulating further exploration of what the author considers to be an interesting and important subject for psychoanalysis.

Relationship of trait anger to resting blood pressure: a meta-analysis.

A series of meta-analyses were conducted to assess whether anger is related to essential hypertension. The present review also considered the relevance of the distinction between anger experience and anger expression, the effect of participant selection bias, and the white-coat hypertension effect for the anger-blood pressure (BP) association. Anger experience was correlated with elevated BP, but the relationship was small and highly variable. When positive effects emerged, both participant selection and the reliability of BP measurement posed interpretational problems. Persons high in anger are not merely exhibiting elevated BP in response to testing, so a white-coat effect is not evident. Being labeled as hypertensive may contribute to higher anger scores, however. The review suggests lines of future research concerning associations between trait anger and blood pressure.

Effects of anger on left ventricular ejection fraction in coronary artery disease.

This study examined the comparative potency of several psychological stressors and exercise in eliciting myocardial ischemia as measured by left ventricular (LV) ejection fraction (EF) changes using radionuclide ventriculography. Twenty-seven subjects underwent both exercise (bicycle) and psychological stressors (mental arithmetic, recall of an incident that elicited anger, giving a short speech defending oneself against a charge of shoplifting) during which EF, blood pressure, heart rate and ST segment were measured. Eighteen subjects had 1-vessel coronary artery disease (CAD), defined by greater than 50% diameter stenosis in 1 artery as assessed by arteriography. Nine subjects served as healthy control subjects. Anger recall reduced EF more than exercise and the other psychological stressors (overall F [3.51] = 2.87, p = .05). Respective changes in EF for the CAD patients were -5% during anger recall, +2% during exercise, 0% during mental arithmetic and 0% during the speech stressor. More patients with CAD had significant reduction in EF (greater than or equal to 7%) during anger (7 of 18) than during exercise (4 of 18). The difference in EF change between patients with CAD and healthy control subjects was significant for both anger (t25 = 2.23, p = 0.04) and exercise (t25 = 2.63, p = 0.01) stressors. In this group of patients with CAD, anger appeared to be a particularly potent psychological stressor.

Visual discrimination and recognition of facial expressions of anger, fear, and surprise in 4- to 6-month-old infants.

On the assumption that the ability to discriminate facial expressions has adaptive value to infants during early social exchanges, ethologically based theorists have argued that this ability is innate. Guided by this perspective, we investigated the ability of infants, 4-6 months old to recognize and discriminate facial expressions of anger, fear, and surprise. Results obtained with an infant-controlled habituation-recovery procedure showed that infants both discriminated and recognized these expressions when portrayed by several adult female models. In addition, infants spent more time looking at expressions of anger and surprise than at fear expressions. These results suggest that infants can abstract configurations of features that give affective meaning to facial expressions. It is suggested that the differences in habituation to each expression might be the result of their distinct functional signification for the infant.
