[New developments in mass roentgen studies].

Abstract

Since 1984 in the GDR the persons older than 40 years are summoned to participate in the mass X-ray examinations in a two-year sequence. The risk groups of tuberculosis are called up at shorter intervals, this also concerns younger people. To this belong patients with silicosis, so-called "healthy carriers of lesions", persons with contacts to patients suffering from tuberculosis, diabetics and the staff of tuberculosis hospitals and pathological institutes. Furthermore, persons working in national education, kindergartens and cattle-breeding are particularly controlled. Patients with chronic diseases of the respiratory tract need our increased attention.

Transthoracic esophagectomy and lobectomy performed in a patient with synchronous lung cancer and combined esophageal cancer and esophageal leiomyosarcoma.

We report a pitfall deriving from the assumption of metastatic disease based upon seemingly identical histology in a pulmonary lesion and in the esophagus. In a 60-year-old patient, cT1 esophageal squamous cell carcinoma was found. One of the two pulmonary nodules was histologically diagnosed as metastasis. When esophageal perforation occurred during palliative therapy, esophagectomy became necessary together with the right lower lobectomy for the removal of the remaining pulmonary lesion. Definitive histology showed pT1N0 cancer of the esophagus, primary esophageal sarcoma and pT4N0 bronchogenic carcinoma. The other pulmonary lesion was re-evaluated and defined as intralobar M1 of bronchogenic carcinoma.

Acrokeratosis paraneoplastica (Bazex syndrome): case report and review of literature.

Acrokeratosis paraneoplastica of Bazex is a rare syndrome. This condition is a distinct skin marker of supra-diaphragmatic neoplasia. This syndrome is important because the cutaneous findings preceed the onset of symptoms referable to the underlying neoplasm by several months in majority of the cases. We report here a case of a 63-year-old cachectic female who had classic psoriasiform lesions of acrokeratosis of Bazex with underlying bronchogenic carcinoma of the right lung.

Minimally invasive pneumonectomy.

Technical advances have resulted in the growing popularity and success of video-assisted thoracoscopic lobectomy for the treatment of nonsmall cell lung cancer. Several investigators have used similar techniques to safely perform pneumonectomies. Minimally invasive pneumonectomy may be indicated for patients who have centrally located malignant lesions and who are not candidates for sleeve resections. Adequate exposure of the proximal hilar vessels is mandatory. Short-term results are encouraging, but prospective long-term data are essential.
